Or if you get knocked out or fall from the Hollow reshaping someone on the outside can physically pull you out.

As for the characters ages Koleda and Ellen are confirmed adults, Koleda is just called a child because she is around 18-20 just like Belle and Wise while Grace and the others are 30-40, at least that age should be the case based on the timeline and statements unless Koleda has taken a dip in her fountain of youth or something.
