  • Paramedic Mike Borrello breaks down Chicago Fire, Trapped Under a Bus, to talk about how real it is. Most shows that portray Paramedics, Firefighters, and Police Officers tend to add a little too much Hollywood. Chicago Fire is one of the few shows that does a half-decent job of making the accidents realistic but still fell short in a couple parts of this scene.

    In being a FF and EMT myself, as well as having friends within the Chicago fire department that work as recurring extras on the show and having been an extra on Chicago Fire a few times myself, the show strives to make it as realistic as possible, with obvious added drama for tv. The writers work with the Chicago Fire department to find out how their scenarios would play out in "real life." That could be why the show isnt as "fake" as other shows. My fav show by far

      Yeah I am a lot more lenient with Chicago Fire because I can tell they try, and learned from buddies of mine who go to IFSI that a few of the main cast go to it to learn and become proficient in the skills so they can do it right and as close to reality as possible.

      @Tennessee country boy the issue is mainly the show runners probably can’t show most of what we do on the daily cause of the studio. Some of these guys even go to IFSI and actively partake in the same classes as everyone else. They also spent a lot of time on ride-alongs apparently as well.
      I know at least one of the actual cast is a member of Chicago FD (Tony Ferraris I think is his name), keep in mind that they probably do have to tone down some of the stuff we do because we do try to push a professional image of ourselves in public than when we are at the Fire Station, and since they are repping Chicago FD, the Department itself may have had their guys act a bit different as well when dealing with the actors.

    I’m one of the stuntmen that works on Chicago fire frequently, while yes they change some stuff for TV most of it stays as real as possible, the tech advisor/ executive producer was once the chief of Chicago. Glad you guys like the show!

    I really like how you talked about how diversity is important and size differences are important because a lot of people think that to be a firefighter you have to be big and bulky and that women can’t be firefighters because they’re smaller but you pointed out that everyone has a purpose and that diversity is important

      When the 70s show Emergency was on tv, the mike stoker guy who drove Engine 51 was an actual los Angeles county firefighter. He had his SAG card (actors union) and was certified to drive a fire engine.

    The moment where they need a smaller guy or gal, that’s super real. Me being only 5’5, I’ve constantly been criticized for my height. however, when I joined the fire exploring program, my advisors have said, it is important to have smaller people on the crew. I like that you break things like that down!

    My mom told me about this video while watching the Chicago series last Wednesday. You're right it's very impressive how realistic this show is but what you may not know (I told this to her in our discussion) but Chicago fire show runners actually consult the real men and women of firehouse 51 and they help provide an authenticity to the rescues

      Fire, med (don't know about pd) really try their best to be accurate (unlike other fire or medical shows) and they often deliver a great result: It's still made by tv directors but they don't let the drama screw up all the medical or firefighting scenarios.
      Which is why I love OneChicago (med, fire, pd)

    Chicago Fire is my fav. I know how they set it all up and all of that. They have real firefighters on scene telling them how to do it. With 911, I enjoy the concept and idea but honestly not the best.

    I love this show because, even though it is fictional, it does a better job than 9-1-1 with making the rescues seem more realistic and attention to detail is better. Great video, really cool to see someone in the field react to this. Thank you for this and your service!

    'Emergency!' will always be my favorite among the rescue shows. It was the 1970's so they could only get away with a little when it came to showing blood. But it was vital in introducing America to EMS. In fact, when it premiered in 1972 there were only 12 paramedics in the country. After it went off the air in 1977, there were paramedics available within minutes to most situations. That's why it will always be my absolute favorite, plus it was entertaining.

      @@HeroesNextDoor Emergency episodes were unknowingly educational programs. Remember there was NO national paramedic standards and procedures; they were local at the time. Notice the different procedures in Los Angeles County, Seattle, and San Francisco ( before the paramedics became part of the fire department). The episode Equipment made fire departments realize that the engine and truck needed paramedic equipment aboard. The episode Welcome to Santa Rosa County help poor communities how to create their own EMT program using pickups and CB radios. The show pushed for CPR training, advance box ambulances and introduce trauma medicine. Plus introducing the K-12 Rescue Saw, the Stokes basket, firefighting helicopters, IV fluids, portable EKG and defibrillator, etc.

    Chicago Fire, has CFD Chiefs and Paramedics, Directing them as they are doing these scenes. So the use of the back board, and such are CFD Procedure, And are being done as the CFD, Has instructed them to.

    In the newest seasons, they have been doing a much better job of putting on flahshoods and not going off air in a IDLH environment...still have a long way to go but seems like they are listening to advisors alittle bit

      Yeah for sure, I really feel like Chicago Fire tries to make it realistic, but they still throw in some Hollywood reality for entertainment. They are definitely one of the better shows and you're right they seem to be listening to advisors, because they have gotten better.

    A very big concept with firefighting that you said is the try before you pry method. Check the door before you break the unlocked door where you could've made easy access through the door

    I enjoy watching a lot of break down or reaction videos by professionals on shows. Just finished watching an episode on Chicago Med on TV, and I've heard that show is pretty accurate so I was curios about Chicago Fire. So typed it into the search bar and found this video, which was only posted an hour ago.

    Great video! I wrote the national exam for Primary Care Paramedic yesterday here in Canada (apparently similar to USA's EMT-A scope). I didn't work any major MVCs during my truck time and we didn't learn a lot about how fire operates in school, so this was a really neat watch
    I plan on working up north in a rural community and was thinking about joining their volunteer FD, but wasn't sure if I should as I'm only 5'2 and definitely don't fit the big, bulky FF stereotype. So, as some other people have said, I appreciate your comments about the need for diversity in the sizes of people!

    Really enjoyed this thanks. In regards to the injection through clothing issue, it is probably more a production limitation because of union rules any kind of nudity has to be contracted and agreed to by the actors, so exposing a leg would require that to be written into said actors contract, which not all actors are comfortable with. Scenes are also often cut down for time, so sometimes we have to assume certain things already took place, as nice as it would be to be able to keep every detail in.

      @@HeroesNextDoor I'm an editor, I went to film school so I'm trained to automatically think of these things. I was on a shoot once where we had to stop filming to change an actors wardrobe because her shirt was ever so slightly transparent under the lights. Production limitations will always mean no show or film can be 100% accurate. I also have a black belt in Karate, so I have the same issues with accuracy when watching fight scenes, if they are more than 85% accurate I tend to give them a pass.

    Being on a Rescue Squad, all the airbags we have can lift vehicles ranging from a small sedan to a semi-truck. Chicago Fire does a decent job in comparison to other shows/films and you can tell that the showrunners are trying to make it as entertaining and as realistic as they can. It is 100% one of the better Fictional Fire Department Dramas out there (especially in comparison to 9-1-1)

    I love Chicago Fire and this was a really interesting video! One of the actors on set, Tony, who's part of the Rescue Squad in the show is an actual Chicago Firefighter which is pretty cool.

    Great commentary, I think given the enclosed space under the bus another option aside from a KED or scoop would be a SKED for a confined space rescue such as this.

    Typically, for a mass casualty event like a bus crashes at the least an EMS plan 1 is called right off, the senior on scene then modifies the plan after initial assessment. A few years ago we had a semi into a CTA station and it went off pretty much like the scenario in the show. I think Engine 8 was first in and they did great job.

    GCS was originally developed for traumas, specifically head trauma. But we tend to use it in medical situations now.

    Im a FF-EMR, we use our backboards quite a lot. Also i have never heard the k.e.d. Pronounced as ked before, that was interesting. One thing ive always wished chicago fire would show is a lucas cpr device. That would be very cool to see them do

    Although Chicago fire isn’t 100% accurate, I would still say it’s the most accurate firefighter show, compared to something like 9-1-1 which is just complete nonsense at least

    LOVE Chicago Fire, have been looking g for a firefighter reaction to it since I came across professionals reacting to various shows.
    Also, by the sounds of things Chicago Fire is quite accurate, think that's why I can't stand other shows like station 19, too much personal drama, with odd fire plots. Chicago seems to focus on fire/disaster(s) of the week and personal drama is secondary/left largely for outside the station. Certainly compared to other shows - has its fair share of on the job personal drama

    I’m not a firefighter but Chicago fire has more realistic scenes than 911 tho, of course sometimes they go too far.
    But in moments of car accidents, rescues, water rescues and tasks assignment when arriving on scene at a fire they’re pretty realistic and accurate
    But there’s lots of things that any first responder has seen in their life.
    In 911 I saw an episode where someone has a spine injury, after they noticed the spine injury they proceed to drag the victim over the floor:-) would never happen in Chicago fire I’m sure

    glad to hear one of my favourite shows is (more or less) accurate! I really value realistic EMS/medical shows, like I only started watching Chicago Med because doctor Mike loved it, which is why I just know I would NOT enjoy 911. EMS people on yt have absolutely slaughtered that one so it's a no no for me :')

    I think one of the reasons why communication between firefighting and paramedic units in general are the result of mixed staging houses. I originally came from Europe where it is not common to house several emergency services at a single location. They usually have every 2 to 3 months a big emergency training session spanning multiple emergency services. But here in the US basically every call is a training exercise. More and more fire departments not only dispatch an EMS to a medical call, but also a firefighting unit for additional support. Breaking down a door, helping with the patient, carrying additional equipment from the ambulance into the house (office), talk to family (calm them, tensions can run high), turn off the stove and all other kind of tasks. One of the things they don't really show on the show is the after action evaluation when they are back at the station.. According to one of the chiefs at our local fire department, this is key why different branches work so well together. They discuss what went great, what could be improved and what should not happen again..

    The only time I ever used a ked as it was intended as a spinal immobilization tool was when I was working for an agency that gave us flimsy cardboard backboards and I didn’t trust it to support my patient’s weight.

    I wanted to watch the show, but I wanted to check if it is accurate since I like to see firefighters' points of view in situations like this. (I know it sounds weird, but I just don't like shows that exaggerated. And One Chicago (fire, pd, and med) is the only show that will keep me watching)

    I think it's so realistic because I heard they actually have a Chicago fireman as an actor. Think he and Alot of actual fire fighters and paramedics truly help them understand how to do certain things

    I think it’s more about the time restraints in one hour shows. They can’t show everything that would normally take hours to do.

    my dad and I watch CF and he is a FF himself, he quizzes me and points out what’s not accurate. It’s overall a really good show but some things sometimes don’t add up

    Valid point on the KED my question would be though, how do you secure the leg straps? My training in NY, its really for extrication from sitting such as a passenger from a car

      good question, we were more thinking of just putting the straps around the chest, and leaving the legs completely out of it, then using the top straps to pull him out.

    Some might say Casey and Brett are communicating a bit too much...
    I love Chicago Fire (PD is good too, havn't really watched Med). Here in the UK we used to have a show called London's Burning which finished almost 18 years ago now (blimey, time flies). I love these types of shows because my Uncle was a Firefighter, retired in 2000 after 34 years on the job.

      Med is also quite accurate, not 100% but still it beats many other shows. Dr. Mike (youtube) reacted on Chicago med once.

    Well since they literally went through basic training with real life cfd you expect a little realism to the show... I admit but enough to be actual fire fighters, but enough to look the part on the show

      all three shows have advisors on board but it's still a drama, it's never gonna be 100% accurate. I still love how they have advisors on board. They had to go through basic training bc they're actually in fire so it's for safety. Always real firefighters on set when the pyrotechnics come out though, usually playing extras. It's fun with PD though cos the background cops can be off duty cops that just, mill around doing cop stuff. Love watching the BTS of these shows.

    Im super fan of this kind of shows and i really love chicago fire because asking to real firefighters i realize that the writters try to make it the most realistic posible 😌
    I really 911 as well buy is a little bit unrealistic.

    Regardless of the fact that they didn't get a current set of vitals if the patient is having a generalized seizure you're not waiting to take a set of vitals on scene until after you stop the seizure. In an active seizure you aren't breathing properly and could potentially allow for brain injury. They made the right move by stopping temperature seizure before getting a set of vitals. Plus not gonna get much of an accurate viral assessment in a grand mal seizure.

    I’ve had a lot of problems with the Chicago series, particularly I remember watching the Chicago med and they had a psychiatric patient in the ER who had a open wound that patient had cut open themselves, the ER surgeons fixed the wound, but then when they left the patient alone and the patient had re-open the wound and the ER surgeon had called for security ER Head psychiatrist got out of line and told the security guards that they weren’t necessary even though they had orders from the hospital’s doctor of security who is higher rank in the head psychiatrist of the ER.
    In the morning it’s on now family is watching they have a hostage situation where they tried to say that the person who is committing a crime had a chance to be free without realizing 18 U.S.C Section 1202, and Section 1203 which makes kidnapping to receiving a ransom or Receiving or Transmitting Ransom a mandatory life sentence, and killing the person who’s been kidnapped a mandatory death sentence in the death penalty states. Note the guy who held people hostage in the Chicago fire actually killed somebody, luckily Illinois not a definitely state, but the majority almost every other state it’s mandatory capital punishment to kill hostage.

    Its a fun show I was so annoyed last night when the premiere was cancelled.. The only thing that gets me about this show is no matter how big the fire in the house it always seems to only be Station 51 responding with no other companies...So that means one Engine One Ladder and the Squad.. The Engine also seems to be the least important in every fire scenario. lol

      Before the recent seasons they had actual firefighters on standby with the engine incase anything went wrong so that's why it seems to be the least of the focus, also to make it more entertaining for TV they probably focus on the companies with the most action so Engine might not get as much screen time.

    So you liked the versed but you didn’t like how soon they gave it , even though the driver had been seizing for awhile?

      They didn’t do any vitals and they stuck it through the clothes, they could of done a better job of assessing the situation

      @@HeroesNextDoor if the patient is actively seizing, vitals will have to wait. Safely assume the patient is tachy, and with irregular respirations; snoring more than likely. So I would have an airway adjunct ready; NPA 28FR ready for placement, pulse ox, ETCO2 NC, O2 prepared. Have a PACE plan set up for airway. I’ll give you the IM through the clothes though. But priority is ceasing and controlling the active tonic/clinic seizure.

    Dude takes his turnout coat off to squeeze under a bus, where there may be glass, leaking oil/gasoline/liquids, sharp edges, hot components, etc. Not only that but he goes under the bus before they've even begun to stabilize it.

    I had a chance to try the scoop stretcher as part of an advanced first aid class. This class was more than 20 years ago, so this is not "New Tech?" Thinking about the past CF episodes, they do tend towards using back boards for moving patients?
    Was also curious--how accurate does this show also represent firefighters during down time? Mainly curious--how much time is normally spent between calls on refining techniques, and continuing education?
    The other thing I've been curious about---the series depicted several cases where individuals wore several hats--e.g. the subplot where Mills was injured, but was able to serve as a paramedic, and Dawson who started as a paramedic, became a firefighter but also filled in as a paramedic when necessary? How close is this to real life?

      The firefighters do a decent job keeping it realistic. Some of the extras are actual firefighters so they know what they are doing. Between calls it depends on what station you are at, most stations have a small gym so they can stay fit. Most volunteer stations have at least 1 or 2 trainings a month so they can stay up to date. Firefighters and paramedics can switch back and forth depending on if they actually have adequate training. A lot of firefighters are EMTs also so they could do some basic first aid and ride on an ambulance. Everything really depends on what your fire department allows and sometimes what your state allows

    It is as real as it can be, what people do not understand these people are actors not professionals, so will never be an accurate scenario of what actually occurs ,however it is a very good series

    Its nice to see a show that makes some effort to be accurate. Personally, I can't watch this stuff because of how they portray the characters. The drama is overplayed and unrealistic.
    As a firefighter I like how you guys show here that not all firefighters are big muscle guys. I am 6'2" and weigh 160 lbs. Even with my heighth I can get into and work inside extremely small spaces.

    Typically those bus doors are controlled by air pressure. Now the controller for both doors in that type of bus and many transit buses is a single lever and is actually just inside the drivers window on a switch panel located there to be conveniently operated by the driver left hand. Its also easy to reach from outside if the window is open. However the rear doors may cease to function if the bus is shut off. The master cut off for the air supplied to the door systems may vary from bus to bus depending on make/model/year. As well the rear door has its own emergency air release valve near it on the inside of the bus for emergencies(a break glass type of box but can usually just be opened). I can see how the rear door may not be opened immediately unless a passenger does it. Clearly the tech-advisors on the show are sharp. Our FD where I live (Canada) actually has a couple retired buses on one of its lots now for training.
