From quest where u should kill Fatalis (its named The End of the Nigh). For the first time you'll get 2 commendation, from the all next times you'll get only one per killing Fatalos.
And u can get 1 by completing Lola's (i forgot her name) quest in the end of the game. U should bring smth from tower of illusion.
