I wish I would have seen this before I had my upper/lower eyes done. Worst decision of my life, changed the shape of my eyes and made my lower face instantly droop. I hate looking in the mirror now. Your videos are the best.
@@Onehotjuly I have! I am still having problems with swelling and sensitivity of the lower and some upper portion of my eyes. The surgery was over a year ago with a double certified plastic surgeon. Not sure if there was nerve damage done.
My heart hurts for you. I am so sorry. I cannot imagine how hurtful it is to have procedures performed to feel better about ourselves, just to be more dissatisfied with the results. It is so odd, because I scheduled surgery for the 28th, to remove the family-curse of undereye bags, and an upper blephoroplasty for my badly-hooded eyes. I have always had small lid space but now in my 60s, a few hours after waking, gravity causes lids to disappear because the flesh is laying on my lashes. What is so odd is that very recently, I have read several posts in which people have had the same or similar work done, and regret it. I do not believe in coincidences and after reading yours tonight, am seriously considering canceling the procedures. I sincerely hope you continue to heal...the swelling to go down...and you are happy with the results.

I am an RN and worked for a group of plastic surgeons. They always took 8x10 photographs prior to surgery and were hung during surgery. I was 22 and we had a 35 year old who was having an upper blepharoplasty and my eyes looked worse than hers. I had very heavy fat pads, upper eyelid was not visible and with many allergies they would get puffier. Despite being well rested and feeling fine, people would comment how tired I looked. Both my parents had similar structures and their eyelids were starting to encroach in their field of vision. I discussed it with one of the surgeons who did it at no cost and had it under local. It was the best decision I ever made and I no longer got the tired comments. It did change the shape of my eyelids, but it improved the look. It’s been more than 40 years and the lateral lids have recently started to droop, but I don’t have the heaviness and puffiness. People who didn’t know me before my surgery told me my son, who was born 5 years after my surgery, had my eyes, which was erroneous as he took after his dad’s side. I told my son as he got older and showed pictures prior to surgery sh he knew that his children might inherit the same trait. He told me that the opened my eyes and made them my best feature. I have never had any other cosmetic procedures and never felt the need as my eyes look better in my late 60’s than my parent’s did in their late 40’s. My sister had the same eyes but her upper lids were even puffier and she was envious of mine. I never regretted the decision.

YES I know exactly what a blepharoplasty entails. In April I had an upper and lower blepharoplasty. Oh my goodness, that was intense. But I definitely look better. I also had a face and neck lift, and an upper lip lift. I spent a lot of money, and it was worth every penny imho.
What was so intense about it? I'm researching because i want to do both around the new year. I'm also thinking about doing something to my lips because they have always been thin but of course with age they are really thin now. I dream of nice full lips to put pretty lipsticks on.
Where did you go? I just need a lower bleph -my upper lids are good and not sagging over my eyes at all- I have only the lower lid issue but am afraid to go to just anyone- the surgeon I had a consult with suggested just removing a little bit of fat from the lower -and tightening the skin up.

This video is so informative, I love how you teach and explain the “why” behind the ”what.” I’m approaching 60 in a few months and have wanted to rejuvenate my face for years. I would love to have you refresh my face, especially my eyes!!! Edit: I’ve heard that allergies can add to the puffy under bag and I would like to see you do a video and explain if there are any steps one can take to help alleviate that. I take allergy meds, nasal sprays, drink lots of water, etc etc but my bags are very baggy 😆 especially in May and October, the height of allergy season where I live.
yes, 100% you hit it on the nail. My mom had a facelift 2 years ago and her surgeon did an amazing job but only cut out the top part of her upper eyelid but she still needs a brow lift and a lip lift. She literally looks 20+ years younger. Amazing but definitely good to understand that eyelid skin can change the way the eye appears.

I had an upper a little over a year ago and was told my lowers weren’t ‘loose’ enough to do yet. So, my lowers aren’t done yet either…It seems more frequent that upper lids are done first. I know a few women who had uppers done years before the lowers…
Great info Dr Karam! Would like to know what is the average time it would take to heal from this type of surgery. I'm a personal trainer so my job is physically active and the healing process may interfere with it. I know it varies from one another but taking a break from my job will be part of the costs of the surgery
It took mine around two weeks but with working out anything down facing like push-ups , downdog etc I could feel pressure in my eye area . It didn’t hurt but did freak me out so I avoided them for a couple of months . Hope this helps
Just had my lower blepharoplasty done in Gangnam, South Korea. Would’ve preferred your office but I’m not making the big bucks quite yet. Very happy with the procedure-it’s life changing.
Dr Karam you make it all necessary to educate before making a decision so informative and makes so much sense. If it was affordable I would jump on a vertical face lift or mini face lift fat transfer. In this time in our economic system makes it impossible speaking for myself, but if I could find a way make no mistake you be my number one choice indeed. I’ve watched all your videos.
It seems pretty clear when you read the comments that even when you see a series of before and after pictures there is no way to know what your surgeon can produce for your face. Even with a lot of research this is a blind item. Very distressing all the way around.
Thanks for this. I was just thinking I'm going to get my eyes done. I've tried a specific brand of mini current that's used by estheticians, with a lot of success. But I didn't get on it soon enough and my lid skin was already apart from the fat and drooping. So there's only so much that mini current could do. Plus, mini current does wear away some of the fat and some of that fat is needed for the under-brow area. So I think my best option is lid surgery. Of course your work is exceptional so I'll be ringing for an appointmemt this year. Your final results are perfection. Not to criticize other surgeons, but I did see a before and after by another top surgeon. Now the before was very saggy and the after did look incredible and a very natural job. No question about it, but for me I'd be going back for a 2nd lift after that first one, just to really lift some of the excess sag. So I'd be doing another before and after. 😇 Some women are just happy with the lift they get, and some women don't want their skin too taut, as they may have become accustomed to a little sag and still want that there. Or they may not want quite a dramatic lift result. Me on the other hand have not forgotten the taut skin of my 30's, and I know if it's pulled my entire face will look 32 again. Dr K seems to dial into a womans wants and is able to deliver a semi-taut lift or a full taut lift. They all look great, but I'm interested in the latter and so far he's the only gallery I've seen that delivers what I want, using an uplift rather than a side pull.

Why all the mystery? Now I understand that my droopy eye corners & hooded eyelids are due to weakness in the muscles at the tail of the eyebrow. I had no idea there are 3 different eye surgeries. I love the way miss Kylie Kardashian eyes look. I heard that there was a mistake that gives her that Disney eye look. the mistake looks fabulous! You do fantastic work Dr. Karam! In my thirties, I had a Blepharoplasty hidden incision where the fats were removed and nothing else was done. I ended up with a deep groove under my eyelid and I had to address it with fillers. Then fillers at that area are not good as they migrate, so I had lumps. Then 2 years later, fillers seem to have gone and my left side fat pads are back and looking great. The only problem is my right fat pad which is still hollow. I also have lower eyelid retraction on it, which I had before the surgery anyways. But surgeon did not address those two issues when done the surgery and he's a famous guy in New York and cost me almost $10k.

I'm having ptosis surgery and blepharoplasty for the upper lid. Having a Dr from Shiley do the procedure 8/30/22. I had another Dr do strabismus surgery this past April. So far so good.
@@tinagrace6722 you'll be fine. Mine was for double vision in both eyes. For that I was put under general and i woke up with straight vision! It was the best experience. I trust my Drs and the care I receive. I went to Shiley at UCSD as I'm from SD. You'll be ok. Focus on the outcome . 👍😇
I have very low eyelids that are slowly covering my eyes! Looks like my eyes will slip off my face any day now! Would love Paris Hilton’s new eyes!! Dr Karam will see me one day for sure.

My mother has drooping upper eyelids so I’m helping her find a specialist, but it’s becoming so difficult to find one as every surgeon makes claims that their technique is unique from others and produces most natural results. Dr. Karam doesn’t seem to endorse endoscopic brow-lift while every oculoplastic surgeon who trained under ASOPRS fellowships all do endoscopic brow-lift with upper lid blepharoplasty and they say that they are better trained in understanding the anatomy and doing the surgery than plastics & reconstructive surgeons. It’s just exhausting and frustrating to seek consults that cost a lot of money and they all say different things. For example Dr. Karam promotes using fat injections for volume while some of his peers say fixed cheek implants are superior and permanent because fat injection shift over the years and problem reoccurs.
Please make sure that she wants to do this. It is a very complex procedure. Doctors make it sound easy but it's not and can cause dry eye and MGD and you do not want to put her through this. Make sure you obtain an ocular surgeon that practices eye surgery. I was going to do both lower and upper. thankfully I did NOT get the upper. I cannot imagine where I would be now. I'm dealing with a very bad decision with the lower bleph because my eyelids are too low now and I'm thousands of dollars in eye doctor bills.
Cheek implants def give you much sharper look, not puffy like with fillers. I did them and I know! By the time they move and go down as I feel with my face
I absolutely love your work. Hopefully someday soon I will meet you. I had fat transfer done to my tear troughs. Sadly, the graft on one side didn't take and it bothers me intensely.

Excellent video Dr Karam. I am wanting to get these procedures done and it is hard for a regular person to explain what you need. With your information I will be able to exactly state what I am after to my surgeon. I only wish you were available in Australia to do these operations. May I also add that I had an upper lid blepharoplasty done in a Dr's room when I was approx. 25 yrs old (now 54). I naturally had a droopy eye lid which made me look tired. The Dr didn't do this as a surgery but just in his office rooms. With just a local injection, he cut away the excess lid and laser'd it back together. The whole thing taking less than an hour and less than $200. I would love to get it done this way again to avoid the high costs of surgery. What do you think Dr Karam about this type of procedure done in a room rather than in the surgery?

I had upper bleph and a unilateral brow lift 12/21. Only local anesthesia, no problems! It was done in Drs office. Honestly, I’d rather not be under general for this….hubby drove me home.
Dear Dr. Karem-help! I’ll be 60 in December of this year. I know that you probably don’t read these conversational notes- but I’m pushing my luck hoping you will. My eyes used to be so bright- but have become very weak and covering the majority of my eyes. Plastic surgery is expensive- and it can be financially challenging. The before-after pictures obviously they look good because they had a total vertical restore face lift. Some eyes look way better, others didn't see much change (as for the eyes only). Definitely the face lift/neck lift it's what gives the most of the change.

Any procedure that can be done for someone with dry eye? Apparently a bleph affects the Miebomian glands leading to dry eyes. So my question is, can someone who has a dry eye condition have this procedure?

The sense of self-betrayal is real-if you get a procedure done poorly, it feels like you've betrayed your own face. That’s why I wasn’t surprised to see that these doctors charge nothing less than $50k-$75k or more
Thx doc! Till I get enough nerve to do this procedure, I have started Sculptra, just 1 treatment has helped! Also, my doctor recommended a “chemical eyebrow lift” to help my drooping right eyebrow, she injected the contour of my upper eyebrow and my 11’a never done this combo before so let’s see how it looks in a couple of days! @@cristinacarrasco5457 I had a couple of small wrinkles but we added CO2 laser around the eyes and i think that’s what fixed the wrinkling.

Edit: I’ve heard that allergies can add to the puffy under bag and I would like to see you do a video and explain if there are any steps one can take to help alleviate that. I take allergy meds, nasal sprays, drink lots of water, etc etc but my bags are very baggy 😆 especially in May and October, the height of allergy season where I live.
yes, 100% you hit it on the nail. My mom had a facelift 2 years ago and her surgeon did an amazing job but only cut out the top part of her upper eyelid but she still needs a brow lift and a lip lift. She literally looks 20+ years younger. Amazing but definitely good to understand that eyelid skin can change the way the eye appears.

I had an upper a little over a year ago and was told my lowers weren’t ‘loose’ enough to do yet. So, my lowers aren’t done yet either…It seems more frequent that upper lids are done first. I know a few women who had uppers done years before the lowers…

Great info Dr Karam! Would like to know what is the average time it would take to heal from this type of surgery. I'm a personal trainer so my job is physically active and the healing process may interfere with it. I know it varies from one another but taking a break from my job will be part of the costs of the surgery
It took mine around two weeks but with working out anything down facing like push-ups , downdog etc I could feel pressure in my eye area . It didn’t hurt but did freak me out so I avoided them for a couple of months . Hope this helps

If it was affordable I would jump on a vertical face lift or mini face lift fat transfer. In this time in our economic system makes it impossible speaking for myself, but if I could find a way make no mistake you be my number one choice indeed. I’ve watched all your videos.

It seems pretty clear when you read the comments that even when you see a series of before and after pictures there is no way to know what your surgeon can produce for your face. Even with a lot of research this is a blind item. Very distressing all the way around.

Thanks for this. I was just thinking I'm going to get my eyes done. I've tried a specific brand of mini current that's used by estheticians, with a lot of success. But I didn't get on it soon enough and my lid skin was already apart from the fat and drooping. So there's only so much that mini current could do.
Plus, mini current does wear away some of the fat and some of that fat is needed for the under-brow area.
So I think my best option is lid surgery.
Of course your work is exceptional so I'll be ringing for an appointmemt this year.
Your final results are perfection.
Not to criticize other surgeons, but I did see a before and after by another top surgeon. Now the before was very saggy and the after did look incredible and a very natural job. No question about it, but for me I'd be going back for a 2nd lift after that first one, just to really lift some of the excess sag. So I'd be doing another before and after. 😇
Some women are just happy with the lift they get, and some women don't want their skin too taut, as they may have become accustomed to a little sag and still want that there. Or they may not want quite a dramatic lift result. Me on the other hand have not forgotten the taut skin of my 30's, and I know if it's pulled my entire face will look 32 again.
Dr K seems to dial into a womans wants and is able to deliver a semi-taut lift or a full taut lift. They all look great, but I'm interested in the latter and so far he's the only gallery I've seen that delivers what I want, using an uplift rather than a side pull.

Now I understand that my droopy eye corners & hooded eyelids are due to weakness in the muscles at the tail of the eyebrow.
I had no idea there are 3 different eye surgeries. I love the way miss Kylie Kardashian eyes look. I heard that there was a mistake that gives her that Disney eye look. the mistake looks fabulous!

In my thirties, I had a Blepharoplasty hidden incision where the fats were removed and nothing else was done. I ended up with a deep groove under my eyelid and I had to address it with fillers. Then fillers at that area are not good as they migrate, so I had lumps. Then 2 years later, fillers seem to have gone and my left side fat pads are back and looking great. The only problem is my right fat pad which is still hollow. I also have lower eyelid retraction on it, which I had before the surgery anyways. But surgeon did not address those two issues when done the surgery and he's a famous guy in New York and cost me almost $10k.

I'm having ptosis surgery and blepharoplasty for the upper lid. Having a Dr from Shiley do the procedure 8/30/22. I had another Dr do strabismus surgery this past April. So far so good.
Excellent I like both those surgeons and send my ptosis cases there.

@@tinagrace6722 you'll be fine. Mine was for double vision in both eyes. For that I was put under general and i woke up with straight vision! It was the best experience. I trust my Drs and the care I receive. I went to Shiley at UCSD as I'm from SD. You'll be ok. Focus on the outcome . 👍😇

My mother has drooping upper eyelids so I’m helping her find a specialist, but it’s becoming so difficult to find one as every surgeon makes claims that their technique is unique from others and produces most natural results. Dr. Karam doesn’t seem to endorse endoscopic brow-lift while every oculoplastic surgeon who trained under ASOPRS fellowships all do endoscopic brow-lift with upper lid blepharoplasty and they say that they are better trained in understanding the anatomy and doing the surgery than plastics & reconstructive surgeons. It’s just exhausting and frustrating to seek consults that cost a lot of money and they all say different things. For example Dr. Karam promotes using fat injections for volume while some of his peers say fixed cheek implants are superior and permanent because fat injection shift over the years and problem reoccurs.
Please make sure that she wants to do this. It is a very complex procedure. Doctors make it sound easy but it's not and can cause dry eye and MGD and you do not want to put her through this. Make sure you obtain an ocular surgeon that practices eye surgery. I was going to do both lower and upper. thankfully I did NOT get the upper. I cannot imagine where I would be now. I'm dealing with a very bad decision with the lower bleph because my eyelids are too low now and I'm thousands of dollars in eye doctor bills.
Cheek implants def give you much sharper look, not puffy like with fillers. I did them and I know! By the time they move and go down as I feel with my face
I absolutely love your work. Hopefully someday soon I will meet you. I had fat transfer done to my tear troughs. Sadly, the graft on one side didn't take and it bothers me intensely.

Excellent video Dr Karam. I am wanting to get these procedures done and it is hard for a regular person to explain what you need. With your information I will be able to exactly state what I am after to my surgeon. I only wish you were available in Australia to do these operations.
May I also add that I had an upper lid blepharoplasty done in a Dr's room when I was approx. 25 yrs old (now 54). I naturally had a droopy eye lid which made me look tired. The Dr didn't do this as a surgery but just in his office rooms. With just a local injection, he cut away the excess lid and laser'd it back together. The whole thing taking less than an hour and less than $200. I would love to get it done this way again to avoid the high costs of surgery. What do you think Dr Karam about this type of procedure done in a room rather than in the surgery?

Dear Dr. Karem-help! I’ll be 60 in December of this year. I know that you probably don’t read these conversational notes- but I’m pushing my luck hoping you will. My eyes used to be so bright- but have become very weak and covering the majority of my eyes. Plastic surgery is expensive- and it can be financially challenging. If you were to ever reach out and offer experimental episodes- I would love very much to be considered. Help from Maryland…
The before-after pictures obviously they look good because they had a total vertical restore face lift. Some eyes look way better, others didn't see much change (as for the eyes only). Definitely the face lift/neck lift it's what gives the most of the change.
If someone has facial laxity addressing the whole face including eyes is better naturally.

Thx doc! Till I get enough nerve to do this procedure, I have started Sculptra, just 1 treatment has helped! Also, my doctor recommended a “chemical eyebrow lift” to help my drooping right eyebrow, she injected the contour of my upper eyebrow and my 11’a never done this combo before so let’s see how it looks in a couple of days! What else can I do for the puffy/fat bags? 🙏🏽🤞🏼🙏🏽
