Working Out with Biceps Tendonitis (DON’T SKIP BICEPS!)

    Biceps tendonitis is one of the most common gym injuries that can keep you from working out. The ironic part about this injury is however that it is likely not your biceps workouts and exercises that are causing the problem in the first place. In this video, I show you the real cause of biceps tendonitis and give you hope to be able to continue your biceps workouts despite dealing with this issue.
    The anatomy of the biceps is an important place to start when discussing the issue of biceps tendonitis. There are two heads to the biceps; one is the short head which attaches to the coracoid process in the shoulder and the other is the long head which attaches to the top of the glenoid or socket of the shoulder.
    When there isn’t adequate strength in the rotator cuff muscles, the head of the humerus migrates upward in the socket any time you perform any overhead lifting exercise. Don’t get confused with the term overhead either. It is possible to be descending through a rep as in the bottom of a pullup and have your upper arm travel up over your head. The important idea is that once the arm travels past shoulder height you are potentially going to have a problem if you have an unbalanced rotator cuff.
    When this happens without adequate strength in the cuff you will have your humeral head migrate too far upwards. This can cause pinching and impingement of the structures that sit just above the humeral head and just below the acromion or roof of the shoulder joint. The tendon of the long head of the biceps gets jammed against the roof of the joint and winds up getting pinched.
    When you repeat this process over and over (as you would with any series of sets and repetitions in exercises that place the arm overhead) you will get an eventual breakdown and fraying of the tendon. The first stage of this is usually seen in an inflamed biceps tendon which results in biceps tendonitis. The irony again is that traditional biceps curls are likely not to aggravate the condition. In fact, by stimulating blood flow to the muscle, you may actually help the situation to speed healing by including curls rather than avoiding them when this hits.
    What you will want to modify however is the amount of overhead pressing, bench pressing or even the position of your arms during squatting. All of these exercises are much more likely to bother a bad shoulder once it is already inflamed or they are causing the pain in the first place. Make adaptations to your training to continue to workout without having to skip arm day all together.

    I had severe biceps tendonitis on and off for the past 3 years and had to avoid push exercises. The way I fixed it: loosen the fascia surrounding your pecs and lats (as well as the rest of your body including your thoracic spine) using a foam roller and lacrosse ball. Do this daily. Also incorporate static stretching at the end. This works thumbs up!!

      This 👍🏻 I had an acl tear in right shoulder, not full separation but was suggested surgery by one doctor. Saw a specialist and now have to prevent the knock on effects including tight pec/thorasic area etc loosened frequently with the ball, pretty much back to near full strength in a couple of years with all of the above exercise mentioned and others. Saves money on massage as well. Small price to pay instead of surgery. Point is, you have to balance it out yourself and keep it going. Don’t loose faith! these imbalances are super common and overlooked by most people.

    Thank you, Jeff. I didn't discover you and your great channel and work until Feb, '23, when I resumed training after 20 years. Out of nowhere I began having pain in my bicep tendon as you explained. I believe it began with doing bench presses with my arms at 90 degrees to my body. I since corrected my bench press form per your recommendation in other videos. Knowing I can resume bicep work is great news. Keep up your stellar work. You're one of a kind in every positive respect.

    Man you are phenomenal ,no one has given such a positive approach to the problem , I have been suffering tendonitis and bursitis from 2 years the pain keep s coming back ,so yes there is no point completely stop working out, ur shoulder will just get more weaker and more pain in the end , thank you

    You should definitely address the DISTAL bicep tendonitis problem. It is an issue that concerns a lot of people and really hard to overcome.

      I had moderate to severe tendonitis felt on the distal end of my left bicep, which prevented me from doing curls on my left for 2 years except for one or two very particular motions. I watched this video and realized that I was sleeping every single night with my arms above my head. A month later, and I can curl as much as I did 2 years ago. Localized pain perception isn't very accurate for much of our body.

    Thanks Jeff! I have this specific problem and it is a matter of awareness. The tendency is to raise the arm/shoulder when actually the contrary move is what is needed. As always, great stuff!

    I was diagnosed with biceps tendonitis as well and I always felt as if my shoulder is actually the culprit. And now I finally understood the problem behind it. No matter what I do I tend to shrug on myright side during workouts and also in daily life. I definitely need to work on this now very hard. I started doing rotator cuff workouts recently so I am on the right track, I suppose. I am so glad that channels like yours provide so much helpful information.

    To anybody out there who's not suffering from any sort of shoulder issue yet, start doing rotator cuff exercises. Do them religiously. This is coming from a guy who's only 19 y/o and is about to have a biceps tendonesis and debridement of the labrum. Shoulder injuries fucking suck, man.

      I had a tenodesis on my right bicep last year in September, but it ruptured 4 days after surgery anyways, freaking sucks. Now I had a dumb looking bicep, weakness and pain on that side, and my shoulder is worse after surgery. I'm supposed to go back for another surgery to try and fix these issues I still have, assuming my insurance is going to pay for it.

    I broke my humerus 40 years ago--the head broke off of the rest of the bone--and shoulder pain in that joint has since be the limiting factor in my upper body training. Your video solved my pain riddle! Thank you! As soon as I started AX-1 I noticed a huge deficiency in the rotator cuff strength on the injury side and started adding extra RC work (from another one of your videos). Now I see why I've been able to push hard in AX-1 with less shoulder pain --the RC work. Brilliant.

    Having tendinitis is the worst thing to happen to me I used to love back and biceps, in fact it was one of my strongest and easiest to train for me. Now It hurts to pick up my phone to my ear for more than 5 minutes I have to be extremely careful when lifting in the gym or anything for that matter and I’ve lost motivation to go at all, even after 2 months of rest there’s still some pain. I’m going to try wearing a brace and start a new routine but man this condition has put a major roadblock in my life.

      i dont train my biceps. they are big enough from pull ups and lat pulldown. couldnt do most of the exercises anyway do to an injury.

    I have it in both shoulders! But I’ve worked thru it and now making gains. I found that the scapula was the main culprit. Stretching and muscle mash worked for me. If I’m not careful it will return but I couldn’t even make my bed by fluffing sheets without extreme pain! Thank god for UA-cam channels like this one.

    i like Jeff's concept incorporated in these training videos, he explains the very foundation of body structure, how bones connected to tissues and mussel. it educated me about how body works while in motion, and look deeper for the root cause instead of treating the symptom whenever i suffer from injuries. these videos made my training more holistically. not sure if you will see this, but Thanks man! will recommend your channel to my friends.

      Harry Haller I got ride of it, Stretch your biceps regularly. Do rotator cuff excercises especially the shoulder push up one, and ice your shoulder that helped the most

      I also got this while bench pressing. Went to physiotherapist who said it was a long head bicep sprain. Said to take load off it for 2-3 and do a combination of slow, light weight eccentric bicep curls, lightweight shrugs, and assisted pull ups. After 2-3 weeks will start a recovery process of working back up the weights slowly over a 2 month period. She said try to ice it every few hours for 2 weeks and avoid anything that irritates it.
      God I hope it doesn't ruin my shoulder, I haven't been able to work chest or shoulders for 2 weeks but hope I can make a full recovery. I really hope this doesn't compromise my shoulder/strength long term. Chest and shoulders were always my strongest muscles.
      Anyone make a full recovery?

    Jeff, I tore my lower bicep tendon while helping a buddy move a giant hot tub. Then a few days later I ended up in hospital for a month, due to a cellulitis infection that made my knee septic and was spreading through my body because it was in the blood. Had to go through surgery on my knee and my calve. Anyway by the time I got well enough it was too late to repair my bicep. So I'm stuck with a torn lower tendon for life. Any tips on making injury get the best it can? Thank you for any advice!

      @@Dispensationalism most of the time if you have tendonitis you are recommended to do surgery under 1 year or under else it becomes harder to operate maybe leading to disability

    I had been training for a year and a half. I felt a sharp pain in my shoulder which put me out of work and training for about 2 months. The doctor told me it was an inflamed nerve, but they rarely tell you what is actually happening. By watching this video Jeff has shown me why I have to pain I do. My biceps have lost strength and everytime I lift something above my head a tightness in my arms and shoulders occurs, so thank you Jeff for not only being knowledgeable, but also being able to assist in worldly affairs. Now I can go train for my strength back because I know what I should and shouldn't do

    Can bicep tendonitis be healed completely? Its stopping me from hitting the gym :(( it really sucks. Would it heal completely?

      ​@@drunkdriver2294 Yes its healed now, I had also started hitting the gym again but stopped due to life. Will continue again next week.
      My Tips -
      1. Don't do bench press, do DB press on ground and lifting your lower body.
      2. If it hurts right now, then don't hit upper body parts. Hit the core, hit the legs but won't recommend doing any other upper body part as it'll make your tendonitis worse.
      3. [IMP] Do dead hangs. This is by far the most important thing, this will balance all the muscle imbalances you have and will create some space in your shoulder so that it can move freely and don't pinch your tendons. This is by far most effective thing to fix shoulder and rotator cuff related problems and do it everyday to prevent any shoulder injuries as well.
      4. Warm up the shoulders every day. Really. Because they get involved in almost every exercise. Don't neglect warm ups.

      @@JamesFeey ah god what a timing! I just got my tendonitis back after a shoulder working 2 days back. I neglected warm ups or maybe the weight was high. Btw it took me around 2 months to fully recover, you should keep doing your rehab but don’t do strength training until you’re fully healed

    This is excellent information. I have bicep tendonosis and came to the same conclusion with a lot of trial and error and documenting of workouts over a decade. This confirms my suspicion and gives me motivation to get back into bicep work.

    I never comment on UA-cam videos, but I can tell you that this guy is truly an expert in his field. His assessment of the kind of pain I'm dealing with is exactly what an orthopedic surgeon diagnosed me with years ago. While I had surgery on one shoulder years ago, I likely need the same surgery on the other one today, but I would like to try and manage the pain first with the right kind of workouts, the kind that Jeff recommends.

      He made one a long time ago. i believe Jeff has suffered from 2 hernias in his life. I think the exercise he said to avoid was a 1 arm bent over row while your knee is on a bench.

    Great stuff - I thought it was my rotator cuff weakness and trying to do a forearm stand.... you not only confirmed it but explained it in a lot of detail so cheers for that and I will focus on strengthening my rotator cuff and scapula work!

    I have been training around my long head bicep tendonitis for six Months. I have a excellent trainer and I can say never give up lifting from a injury. I have of course lost strength in my overhead press but, I would rather lose strength in one area than to give up completely. In three Months from now I will be back to benching 315 for reps!       Tons of rotator cuff exercises, lower lat exercises, rhomboid exercises, mid trap exercises, serratus anterior exercises, thoracic extension mobility, pec mobility, lat mobility, rotator cuff mobility, tons of shoulder stability exercises, subscapularis mobility, upper trap mobility, anterior Delt mobility
      Minimize presses; all aka avoid pressing movements for a while. Perfect form in everything, including the exercises and mobilization’s I gave you
      Balance these exercises with some back.

      Yeah, same here. Ever since I modified my workouts to include more focus on the long head of my bicep & brachialis my weaker arm has been sensitive but not outright painful. Been doing more static stretches after the workout, and it's getting better. Also been using a foam roller on the upper part of my brachialis, and it has helped but been dealing with it for 3 weeks now.

      +Danny Hermosillo Pretend like your doing a curl and your at the top position and keep it there, the is a cut that goes all the way down the outside of the forearm and it hurts that entire line when I curl heavy

    Man I've had bicep tendonitis at my elbow for over a year now. Definitely a back or shoulder problem. Totally using my bicep and upper trap to stabilize my shoulder instead of scapula muscles. Got it when I fell in love with rock climbing and did it 7 days a week 8 hours a day. It's so depressing trying to solve. Physical therapists are horrible. Like 7 out of 10 are wrong and don't understand proper movement patterns and how to turn off my upper trap. Wish I could solve it, but I am lost.

    Jeff, do you have any videos on forearm pain once releasing the bar/dumbbell after a bicep curl? It feels like shin splints, but in the ulna (I think). If I do a medium to heavy set, there is shooting pain upon releasing my grip in the forearm. I try to have the best form and avoid straight bars, and use many dumbbells for bicep curls. Even if I do biceps once a week, with medium sets, the problem persists. I do forearm specific work too, and I be sure not to over-train. I also make sure I don't "curl" at the wrist during bicep lifts. I can do curls in most near perfect form, and the problem will still persist. "heavy" curls are up to 40lb dumbbells for me. Medium would be around 35 or 30's. warm up to light ranges from 20's to 30's. I really want to start grabbing the 45's, and do more 40's, but the shooting pain is unbearable. Even with the EZ curl bar, the problem comes during the medium or heavy sets.

      You should use an ez bar with a more slightly pronated grip than the grip you would use on a straight bar. Works for me

    TY Much for all your Knowledge!! I just went in for, what I thought was, A Rotator cuff issue. After Speaking with a good PT friend of mine and the initial consultation, I Do believe it is the Long Head Bicep. After watching this video, I pretty much KNOW it!       keep back straight , kick ur ass and hips back first then do the actual squatting. your knees should be in a straight line with your toe tips. not going beyond 90º can help too. if your knees hurt while doing hacksquats or leg presses i would Pause leg Training and if the pain is gone you can slowly work your way up again.

      I would suggest backing off a bit, instead of doing entire squads at once try taking the guys one at a time. Then work your way up from there. In a matter of weeks you'll be taking on whole squads again with no pain at all..... well mebe a slight pressure but that's normal.

      I have that too Mehdi. I find using a patellar band helps. Plus lots of stretching of the quads and foam rolling the area.

    Diagnosed with this recently as a baseball player and this was in my feed. Very nice. I was prescribed with slow controlled eccentric curls and straight arm punch outs and a supination to palms up then slow and controlled down keeping the arm straight

    Thanks for this, Jeff. I am one of the unfortunate sufferers of hereditary arthritis. In every joint. Under my acromion process are osteophytes that tear away at the bicep tendon and subscapularis over time. The right side already fully detached (both tendons) years ago, and it changed the way I trained, but it was not devastating. The thing that saved me was the strength and flexibility of my rotator cuff. The main thing to do is to keep the shoulders flexible and pumped with blood. Use ice and rest as much as you can. Also, being aware of your push movements and the torque on your shoulders when performing abduction/deduction-type exercises. I am dealing with the left side now and it is frustrating, but I will consider it a challenge.

    Complete proof that Jeff knows exactly what he's talking about. I recently recovered from bicep tendinitis due to elevated stress from both work and training. The only bicep exercise that bother it was the incline bicep curl.

    Damn Jeff, you are the best. I'm having problems with my left shoulder and my physiotherapeut doesn't know what the problem is. After looking on your channel i start to suspect my Biceps Tendonitis since im very very sore "under" my left front delt after pretty much any push-excercise. This has seriously halted my training, motivation and healthy habits due to the pain wich just doesnt go away. I'm 32 yr old and have had this for the last 2 years....
