Provided that the bmp is done till apex.
And good coronal seal is achieved.
Not at all. Obturation is not a critical step 🙏
If bmp is short how we can correct that before obturation in necrotic canal
Increase WL and do BMP again
If there is a v small discharge after using paper... As the tip of paper shows a bit wetness... Does it mean we need to clear canal again.. There is presence of infected pulp tissues yet...

Plz also tell the use of intracanal medicament like calcium prior to obturation like after bmp it should be used in all cases or selective ones as I think it eliminates post obturational pain by forming a barrier as it's a inert material.

Calcium hydroxide is best intracanal medicament. It must be used in necrotic canals for 7 days for better results. In vital cases, single visit RCT can be preferred.

Sir, in 1st or 2nd visit my patient has severe pain on filing, not letting me go in orifice even after infiltration and block, what should I do then? It doesn't let me do good BMP :(

In first case should we insert the file till full WL or 1 to 2mm short as I have seen in my cases rest of canal goes well but apical 1 to 2 mm of the canal sometimes pains. Is it because of insufficient bmp or this could be due Peri apical vitality?

I just completed RCT of 46(necrosed) but after obturation pt is complaining of pain , I have done everything maintained apical patency with 10no final and irrigated with ultrasonic scaler and there was no pain on placing 20no file..
What can we do now
Cause of flare up is pushing necrotic debris in periapex.
Now manage it with medicines
2 % taper bmp is good or 4&6% I have seen old rct's which has 2% taper n pt has no complain In Treated tooth
Taper will depends on canal. In anterior teeth, wide canals can be prepared with 4% taper.

Minimal preperation is good but cleaning should not be compromised 🙏🙏

No need of antibiotics in irreversible pulpitis cases. Usually you can give painkiller in all cases.

3-7 days for water based calcium hydroxide

Hii sir after preparing canals with protaper hand file or by rotary after this when we put small file we feel stop or a floor like feeling at apex is it normal ? M asking about preparing canals at full wrking length.
You need to do patency with 10 k file in between rotary files. 🙏🙏

Sir maine 3 months pehle root canal krwaya tha jis me rootcanal ke pehle gms pr infection ka ball jaisa hogya tha but after root canal wo chlagya or maine 3 month tk cap nhi lgaya after 3 month maine cap lgwane ke liye gai to waha mujhe small ball jaisa feel hua jo k touch krne pr feel hota hai or use press kro to jis daat me rootcanal hua hai waha pressure feel hota hai doctor ne 2 baar xray kiya or kaha k rootcanal perfect hai usme koi issue nhi hai or cap lgadiya ...but mujhe waha pain to kuch khaas nhi hai but boht irritation feel hoti hai to ye kya problem hai plz btaiye🙏

But doc dnt u think that in pregnancy case (2nd trim) even after preparing absolute BMP ,patient experiences abit pain near to the apical region due to hormonal flare up
Maybe hormones can play some role, but I never faced any such problem in pregnant females

I think some tissue is remaining in canal
