Well if your on Pacifist and everything had Karma affect it would prob be easier cause like it’s called the “Karma” effect for a reason, meaning it probably actually would heal you since you have no LOVE in Pacifist run.
Question: what if you do a true pacifist route, but come to Sans with 19 LV? Does it initiate the fight, or does it require you to actually slaughter everyone to get a bad time?

Karma should deal more damage depending on the LV you are. For LV 1 you Don’t get any karma, for lvl 2 you barely get karma, and the more the lv is higher the more damage KR does.
The lone Karma effect is how I've believed the way karma would work Sans has frameskipping capability because his fight is kinda in a way ..based on him cheating the system?? So Karma is just another one of your toys used to make battles even more unfair for those without their own cheats So basically skeletons (and probably Boss Monster's when not holding back so much) can just remove I-Frames, I guess within my headcanon

Without those I-Frames, I can't imagine the rage from the boss battles from Undertale streamers - especially from the Genocide Undyne boss and Sans boss fight 😂😭💀
This actually puts the Sans bit of "only deals 1 damage" into perspective. He deals 1 damage, every 'tick' you're inside his attacks. Giving the Karma ability to anyone else makes it more of a bad time since they have higher attack.

I feel like this could be canon, since KR ISN’T truly limited to Sans, it’s an effect placed on you after you’ve killed most of the monsters. So, if you fought Asgore in Genocide, he would have KR as well, same for if any other monster replaced Sans.
Toriel and asgore should've had a burn effect, and if papyrus had a phase two then it would be cool that when you got hit by a bone you would instantly turn blue
