    Hi doc! About a week ago, i got a really bad tear on my knee while practicing martial arts.. After jumping, I landed only on my right knee and it bent inwards along with a popping sound. I still can't bend it more than 70 degrees and the swelling hasn't gone down. I'm still unable to walk on the 7th day, since it still feels really unstable. Are these symptoms a grade 2, a grade 3, or perhaps even other ligaments like the acl that got torn? thank you!

      Hi MorcaH. Sorry to hear. May be ACL. May be meniscus. May be collaterals. Best to get assessed sooner rather than later. Good luck. Thanks for watching and subscribing if you did

    It’s crazy how doctors never want to give you an MRI, they rather guess a diagnosis

    • @TalkingWithDocs
      @TalkingWithDocs  4 роки тому +20

      Hi Onboarddatracks. So often the diagnosis can be made from a good history and physical examination. An MRI is not required to diagnose a torn MCL. An MRI would be required for other pathology - like to confirm a torn meniscus. It is not that your doctor doesn't want to order an MRI but rather they want to order tests that will help with the diagnosis and treatment. Hope that clarifies things. Thanks for watching.

      Mine made me wait 3 weeks for an MRI . Knee swollen, loud pop when injured. And he’s like, we’ll wait to see if it heals🤔. Turns out meniscus tear, swollen mcl and torn ACL after MRI. And that was because I went to a different doctor

    Great video - thanks! Here's my follow up question....I've had severe pain in the MCL area for over three months. By way of background, I've played rugby for over 20 years, ran marathons, avid bike rider etc. in other words I've been pretty hard on my body. I did have the knee in question scoped 25 years ago - and since then have not had any problems. Going back approximately six months I ramped up my trail running and biking. I did not experience a typical injury - i.e. fall, twist, side impact etc. One morning after a hard workout I woke up with tremendous pain. It has somewhat dissipated - but not much. It's tender to the touch and the slightest lateral movement is very painful. I have not been to a doctor due to an administrative nightmare with my insurance. My question....does this sound like an MCL tear? or does it sound more like a tibial plateau fracture as mentioned? arthritis? Any insight would be greatly appreciated. Mahalo!

    • @TalkingWithDocs
      @TalkingWithDocs  6 років тому +3

      Hi John. Very difficult to give an answer without examining you. It could be an MCL but would be quite unlikely without some type of injury. It would also be uncommon to be a spontaneous tibial plateau fracture. It is much more likely related to a torn meniscus or arthritis. This could be ruled in or out by a good physical examination and followed by an xray +/- an MRI. Hope that helps. Now, Mahalo - are you watching us from Hawaii?

    Hi, thanks I enjoyed your video. I have a personal trainer at the gym and have been doing leg presses, extensions, calf raises, body weight squats with no problem for some time. A couple of weeks ago I did some fairly heavy leg presses and we also did something akin to a lunge but stepping backwards while holding on to ropes above me for balance. It went fine at the time. I also did some step-ups and side-to-side step-ups which may have put lateral stress on the knees. No pain at the time but a few days later my right knee started to be somewhat painful when standing. It gradually worsened and after a few more days, I stepped on my right foot and heard a pop or snap and immediately felt strong pain on the inside of the knee and couldn't put weight on that leg without a lot of pain. I went to urgent care and had an x-ray which was normal although they did mention I have reduced space on the inside of the knee, but it doesn't look like bone is touching bone at this time. There is no apparent looseness of the knee and the doctor said it was likely an MCL sprain or tear. They gave me a brace (not hinged) and crutches and said to wait a week or two and go to a specialist if it wasn't improving by then. It has been just over a week and I have been icing and resting the knee and talking ibuprofen. I roll around in my desk chair with wheels most of the time to get around. I have also been doing some range of motion exercises such as bring my heel back to flex the knee and pushing down lightly with a rolled towel under my foot. Yesterday, the pain had subsided quite a bit and I could take a few steps without pain. Then I walked outside without the brace on and must have pushed it too far and now they pain is worse again. My questions are how long is reasonable to wait before seeing a specialist, and can I do any walking with the brace or should I be keeping weight off of it? Thanks

    • @TalkingWithDocs
      @TalkingWithDocs  2 роки тому +2

      Hi Keith. So it really depends on your diagnosis. The other possibility is a torn meniscus. The treatment is very different. Usually an MCL would be full weight bearing in a hinged brace for 4-6 weeks. If it’s a torn meniscus it would be weight bearing as tolerated and see how it goes. If no improvement MRI or arthroscopy. Up to you when to see the specialist. When you are frustrated we guess. Good luck. Thanks for watching and subscribing

    Hi Doctors. Thanks for the incredibly helpful video. I am suspecting MCL injury from soccer got excessive force sideways from the inside to outside with symptoms of swelling, popping sound during the injury, knee instability. I have a history of ACL reconstructions in both knees. I just went to see the GP who was not very helpful. I am planning to get an MRI scan and visit the specialist. However, you mentioned that MRI may not be necessary with MCL injury. Do you think clinical examination will be sufficient to determine the degree of the injury? or would it still be better to visit with the MRI data considering my recurring ligament injuries? Thank you!

    • @TalkingWithDocs
      @TalkingWithDocs  5 років тому +1

      Hi Min. It is difficult to answer specific questions without examining you however more commonly if you hear a pop and have instability you may have torn your ACL which is commonly associated with an MCL tear in addition to a torn meniscus. Yes examination and MRI are the two best tests. Good luck and keep us posted. Thanks for watching.

    47 year old active male in overall good shape. Was surfing 7 weeks ago and I jumped off the board at the end of the wave and assumed the water was waist deep but was actually more like knee deep. Jammed my knee. I felt some pain but wasn’t severe. I was actually able to surf for about 30 more mins. When I was walking up to the beach I could tell I hurt myself. Later that day I felt mild discomfort and inner tightness / swelling but it has never swollen to a degree that was visually obvious. More so if I bend my knee all the way if gets a little puffy.
    I took time off all physical activity and did the ice routine for a couple of weeks along with some Advil. I also started wearing a neoprene knee brace after about 2 weeks. I assumed it would just heal on its own but 7 weeks out and I’m still having issues.
    I have noticed that if I press and manipulate the medial side of my knee - where the MCL is I think - it still is sore to the touch. I can extend my leg without pain but if I bend it all the way back I feel tightness and mild discomfort.
    Sorry for all the info. Does this sound like a MCL injury? 7 weeks is so long ! Im planking on going to an ortho clinic in the coming days.
    Thanks

    Hello. Thanks for the vid. Very helpful. Like the banter too. I have a question (if you are answering); I just spent 2 days on uneven ground (boat) no accident- but the MCL part of my same knee is very very painful. I can walk 1/10 pain but cannot bend it with weight (up and down stairs) I can bend it gently without weight fine. IS that the MCL...? I am 47, 5'4" 140lbs. sprite. I had an ACL reconstruction 2 years ago - skiing 🙄 - no jeans 😎. Recovery went all great. No problem with it.... but I am a little paranoid. At what point should I go see someone. RIE right now. THX !!

    • @TalkingWithDocs
      @TalkingWithDocs  3 роки тому

      HI Sea Son. Very welcome. So it could be your MCL but if there is pain with flexion while weightbearing and it is at the joint line it would be more common to be a torn meniscus without any trauma. Only way to know for sure is to get examined and possibly order an MRI. Best of luck to you. Thanks for watching, subscribing and sharing with someone if you did.

    Hi! I was playing volleyball, went up for a spike and came down on my left leg and then i heard a LOUD pop.. tried to put weight on it and immediately felt a shifting towards the medial portion of my knee. I cannot completely physically straighten or bend my knee, I also have a lot of pain when trying this. I had gradual swelling and bruising overtime. I went to the doctor, for assessment and no ACL year was suspected. Doctor gave me 2 weeks off from work as I perform a lot of weight bearing, walking, standing, and lifting for my job. Came back in for a follow up, doctor assessed that my knee is starting to bend (slouch) medially. Could this be an MCL tear, or meniscus cartilage tear? Thank you so much for posting this wonderful video it’s very informative and helps to give us viewers peace of mind!

    • @TalkingWithDocs
      @TalkingWithDocs  3 роки тому

      Hi Antonette. So it is hard to say the exact injury that you have but yes that mechanism can cause a torn MCL, a torn meniscus or a torn ACL. The pop is usually associated with an ACL tear. The only way to know for sure is with an MRI. Hopefully you keep getting better. You are very welcome. Best of luck and keep us posted. Sorry for the delayed response - thanks for watching and subscribing if you did.

    U guys r HILARIOUS!!!!! I fell to the side on my mt bike and my peddle clips were too tight so twisted my knee. Here in CANADA it takes about 2 weeks just to see a dr. I've waited one week so far. My knee is HOT, swelled and medically bruised, I've continued my biking and hiking, icing it. Limping still. Tender medialy. No increased mvt upon valgus test. So with that info, would your guess be grade one sprain?

    • @TalkingWithDocs

    Hi doc, during basketball, i planted my right foot in front of me and my knee popped left. I immediately fell to the ground and i could feel that i knee wasn't stable. I went to the ER and they braced my knee with a brace that limits any motion of the knee. After wearing this brace for 3 days and using crutches, I went to the doctors and they did a physical examination. The doctor then said he was pretty sure that i hadn't torn anything so they took away the brace and lightly rapped it. Now after 4 days, my knee is a lot better with no swelling and no pain but it still feels like it's out of place. What does this mean? Thanks.

    • @TalkingWithDocs
      @TalkingWithDocs  5 років тому +2

      Hi AB JW. It is hard to diagnose your injury without examining you however a physical examination and imaging would typically be able to rule out a torn ACL or MCL. Usually as you regain your range of motion and strength, the injury will declare itself. If you have an ongoing sense of instability, you should be re-examined. Hope that helps.

  • @ttmustin
    @ttmustin 5 років тому +1

    Is there a trade off between stiffness and the speed of healing? Some guidance indicates that immobilizing the knee will speed the MCL healing process. Your guidance indicates that knee mobility during healing is important.

    • @TalkingWithDocs
      @TalkingWithDocs  5 років тому +1

      Hi Mike. It is always a balance but if you have instability due to the severity of your injury, it is critical to immobilize and let it heal before strengthening and working on range of motion. Hope that answers the question. And thanks for watching.

    I am 66 and in pretty good shape. I have always done some daily form of exercise. About 12 years ago I injured my mcl in right leg. When I do too much or certain yoga moves pain flares up. I’ve been doing a group weight training class for about 6 months. Also doing a Balance/Flex class that’s a cross between yoga and Pilates. I’m frustrated because I try so hard to stay in shape along with healthy eating and lifestyle. I’ve spent the last 3 days in bed and icing due to injuring it again! I’m wondering at what point do I start doing rehab type of exercises at home? I want it to heal correctly but I also want to keep strength and a range of motion. Thank you!

      Hi sandra. Ideally you should be assessed and have your diagnosis confirmed. Once the initial pain has subsided rehab exercises can begin. You also want to ascertain whether or not you require a brace of any sort. Good luck and thanks for watching!

    I fell off from my bike two weeks ago that got my left leg stepping on to the ground and twisted a little bit clockwise. I put on compression bandage for 12 days and now on a knee brace. There is no swelling anymore. I can do ducky walk and ducky squat but I cannot stand with just one injured leg. Also it still cannot be fully bent and straightened. I went to the orthopedics immediately after the injury. They took X-rays and did all sorts of tests and said that I just got a sprain. Am I just worrying too much as I thought two weeks a really long time already. I will have a follow up appointment next week.

    Hello Doctor. First of all i love your videos. God bless. I hope you answer me. I hurt my mcl (2nd grade) in february 15. the Pain is almost gone i only feel it if i train leg day in the gym. But when i sit down on my knees it still hurts a bit on the inside. And i also feel a little instability when i lay on the side and lift my leg up and swing it down. So i think its not healed 100%. But 6 months past now. Do you think or would you say this ist normal and it will get better or is there something like it always will be instable. I would be very happy about an answer. I popped my knee pretty bad sparring in jiu jitsu. I've tweaked them both many times with just some pain for a few days/weeks, but this one was much worse with a defined audible pop. Happened on rotation of knee towards inside of leg with force from sparring partner extending the rotation. Pain when I palpate just above knee on inside, feels like right on the ligament. Can actually feel the ligament to touch there whereas doesn't seem to be as pronounced on uninjured side. Can bear weight but quick change of direction or rotation isn't good, and if I stand on the uninjured leg and raise my leg inward with my hands (painfully of course) when I let go I'm unable to support it falling back to the ground very much along with increased pain. Pretty sure I need to just ice/rest so I am waiting out going to get diagnosis for a few weeks on advice of a PA friend. But I do see the value in physical therapy if I'm not going to go in. I was wondering if you can share any resources that you would personally recommend for PT.

It swelled up a lot but now it’s gone down a lot but still slightly swollen. There’s still stiffness there but I can bend it with and without the brace I just can bend it fully how I want without thinking I might make it worse but I can do certain leg workouts and do the stationary bike. Greetings and thank you for the information. Our 16 year old daughter sprained her MCL for the 8th time in the past three years this past weekend. Generally grade 2's. We let it fully heal every time, yet it keeps happening. So far this is the 3rd time in 2021. Very frustrating, as she is looking to play collegiately. Is there such thing as a loose MCL? If so, is there a surgical fix? We saw Internal brace, etc, but don't know those are only for complete tears. Thank you.

    Hello, i fell on the ice on 2/12 and injured my left knee Anyways, I’m just trying to get more feedback. There wasn’t much swelling and while I can walk on it, it gets extremely painful after a while and I have to sit down. It’s also very painful when I sit down for a long period and try to get up. It feels unstable. The range of motion has improved but it’s very painful to stretch it out fully or try to bend it in. It feels weak and unstable at times particularly when I pivot or turn. That’s when I hear crunching/popping back and forth. Bending the knee in causes a lot of pain. The pain is all in the back of the knee. My doctor says it is possible there are partial ligament tears. No MRi ordered at this point. X-Ray was negative. I have an appointment with a specialist on 4/5. Any thoughts? Just looking for second opinions.

    • @TalkingWithDocs
      @TalkingWithDocs  3 роки тому +1

      Hi Jason. It is difficult to say without an examination. Having said that, if your main symptom is instability it is possible that you tore your ACL. If you have pain on the inside of your knee that is worse with bending and twisting it is very possible that you tore your medial meniscus. An MRI will tell the complete story. Best of luck to you and keep us posted! Thanks for watching, subscribing and sharing if you did.

    I think I've got MCL (guessing by the location, description and symptoms), but I got no injury (I didn't get hit, didn't fall, etc. Just woke up one normal morning and had an MCL sprain). How is that possible?

    • @TalkingWithDocs

      Hi Silver Star. It is possible but much more likely that you woke up with a torn medial meniscus. Similar location. MRI to confirm. Thanks for watching and subscribing

    Playing basketball and stopped quickly. I felt my right knee move out of place but no pop or sounds. I got up - stopped playing and walked around a bit to walk it off. At one point oil twisted when walking and felt my knee noticeable - then walking up a step at home my knee fave out. That was 9am. I went to an orthopedic dr at 3pm. No real swelling, no intolerable pain, no bruising. Told her all that happened she took an xray - all clear. She tested for torn ligaments. She gave me a good brace with rods on each side. Told me to come back in 6 weeks - if I still wanted too. Didn’t think I needed an mri now. She seems not concerned that I tore anything. Your thoughts?

    I'm going to the physical therapist for an assessment in a few days. I'm not sure if I have injured my MCL, or torn my meniscus. I'm not an athlete, my injury was I think due to plant-and-pivot type events, twice within a couple of weeks. I both heard and felt a pop, and immediately had extreme funny bone type pain on the inner side of my right knee. After a couple minutes I could weight bear and there wasn't long lasting pain. But as time goes by, it's now about six months since the first injury, the pain has gotten worse and worse. It sometimes feels like a nail is being driven through the side of the knee; walking is very painful, going up and down stairs is painful. What do you guys think it is?

    • @TalkingWithDocs
      @TalkingWithDocs  5 років тому +1

      Hi Lana. As we always say, it is very difficult to diagnose anything without a proper examination and investigations. Having said that, generally speaking, if the pain is sharp and stabbing and along the medial joint line, it certainly could be a torn meniscus and less likely a strained MCL. This may require an MRI to confirm or rule out. Physio is always a great place to start within the limits of your pain. Good luck and thanks for watching.

    Hi,
    I have A question - could ir be MCL tear?
    This started like when I was 14 when I simply threw A ball in the air - I sprained my knee and fell. Now im 26 and now and then about 2 times A year my knee sprains - sometimes from dancing, sometimes by just walking. It just sprains, i usually fall On the ground, after that it hurts and swells On the inner knee side for like 1-3 days.
    Could that be something serious?
    I used to play volleyball a lot in high school and went to doctors when i was little, but the doctor didnt say anything much and i also didnt remember almost anything. He just said eirher live with it, either they will operate, but there could be complications.

    Hello, great video! I had my knee bend from outside to inside during a sporting activity. Primary doc suspects MCL injury. I'm 2 weeks in and getting a little better but still mildly swollen or has some fluid and cannot flex back very far, just a little more than 90 degrees. Could this possibly be due to swelling/fluid or is this more of sure sign of meniscus damage also?

    • @TalkingWithDocs
      @TalkingWithDocs  6 років тому +1

      Hi cpro48609. Thanks for the positive feedback. With respect to your injury, sometimes an effusion (or fluid inside the knee) can limit flexion in the knee. It would be more common for pain to be the main limiting factor. This can be related to the MCL strain or possibly due to a meniscal tear which is often associated.

I was walking fast and it happened in a sec, felt like I pulled a muscle around my knee .It was so painful that I couldn't put my foot on the floor. I'm resting for 12 days now, I'm taking Diclofenac pills, applying Voltaren and (Ice only the first week ) .I can see the improvement everyday , less pain but still can't walk properly , and today I noticed bruises on upper thigh and I'm a little concerned. At ER , the doctor didn't do any X-RAY ,he just examined it and I don't have any broken ligaments or broken knee. My diagnosis is : Knee derangement syndrome . Can I please have more information about this ! Do I need to do MRI !! IS that called a sprain!! And can I start doing some simple exercises?? Please and thank you so much for all your information ❤

      So return to sport should be a decision that you make with your trainer. We typically tell people 90% strength, 90% range of motion and minimal to no pain for return to sport. A hinged brace with help protect your knee if it is in fact an MCL injury.

      Once I was in gym class, I was doing the high jump and it was my turn. I run up and jump and I hit my knee on my forehead. When I was on the mat I couldn't move it. It was so painful. I was limping for 20 mins then I could walk but really hurt. The next week after days of sitting out in gym class it felt okay again. Only this time. I noticed a bump on my knee. It seemed like swelling but it was not. It was solid. That's when I came to the realization that I may have relocated my knee joint or my patella was damaged. So next gym class we were doing 100 meter dashes. My knee felt okay so I decided to run. When I ran felt okay but when I land my feet back down for the 5th time it hurt and I fell over. I went through a cycle of it healing and getting damaged for the rest of the school year. Now in the summer break it healed completely on its own but the bump stayed. My question is, will I be okay or will I need medical attention.

    I have injured my right knee and have seen an orthopedic surgeon. He believed it was my acl but after finding out the fluid in the knee was not blood he said maybe it’s not the acl.I am waiting to have an mri so he will be more certain but it will be a while before I’m able to have it done. I am curious to know what could be causing swelling, feeling of bones rubbing together lightly from 15 degree bend to straight, and very light intermittent pain on medial side of knee. Any help is appreciated, thank you!

      Hi Luis. It is difficult to say for sure without examining you and your imaging however a loose body, a torn meniscus or an MCL tear could cause some of the symptoms you are experiencing. The MRI will definitely confirm or refute this. Good luck while you wait.

      @TalkingWithDocs  5 років тому

      Hi its-WreckZz. It is hard to diagnose the problem via the internet however the good news is that if the only problem is a sound, we typically tell people not to worry about that. As we age, often our knees will make clicks and grinding noises but if it is not associated with pain, it is probably benign. If you develop pain again, that may need to be investigated. We usually advise people to slowly return to the gym and let pain and common sense be your guide. Hope that helps. Good luck! And thanks for watching.
