- Circumduction is a common walking “problem” where someone will swing the leg out to the side to prevent the toe from catching on the ground. Several things can cause this such as foot drop, spasticity, a “stiff leg”, and/or weakness due to a brain injury, stroke, multiple sclerosis and a variety of other neurologic injuries.

Dr. Tobias, thank you for this video. It addresses my needs and explains so much. MS has caused muscle weakness and a buckling leg. To compensate, circumduction and a stiffer gait keep my leg "usable." When exercising, the same leg has trouble bending, as though locked from the hip to the knee. With an adjustment to shift my weight, the leg unlocks at the knee but reverts to buckling if I'm inattentive. Would you say that spasticity is causing the lock-up between my hip and knee? Can that be remedied?

Hello Dr Tara , videos you shared are amazing. Practiced a few , can see a lot of improvement. Gratitude you angel. As i have lesions in my spinal area , my hip muscles are weak. I still initiate movement from my hip . Please guide me how to overcome the same. You are blessed with humanity ❤❤❤

I like your videos its quite helpful. I have spastic foot and the one thing really help is dragging heel on the floor relaxes my calf muscle and that i learnt from, you are pretty smart I tell you. in the morning or when i lay or sit for too long my spasticity increases any tip for that?

Hi Dr. Tobias. I have foot drop due to MS. I have been running marathons with an AFO since 2008. I have disease progression (right thigh weakness and fatigue) and I now have heel whip on my AFO-wearing foot. This is causing knee pain on long runs. Do you have set of exercises for this problem? Hi Michelle. Thanks for watching! that is an excellent question. Remember, your brain is learning what you practice. If you hip hike while doing the exercises, that is how your brain is learning that movement. 😊 that being said, I would work with a PT who can help you figure out why you are compensating with a particular exercises and help you find an alternative exercises.

When I'm sitting in a chair can I cross my stroke effected leg over my strong leg to turn my hip inward and to help correct any circumduction while trying to walk normally again?
Dr.T- I notice this once my leg gets fatigued. Most common on the front/top of my foot. What can I do about that? I do those toe taps every day but maybe that isn't an effective exercise for me.

Hi Monique! First, I always love unrelated ;). Second, the core max might help. Check out the playlist "core max exercises". I have some arm exercises there. If you can get your arm into the positions that I show, than yes, I think there is some value there. :) I hope that helps!

I have ms, circumduction, no foot drop but have had time with stairs. In some ways down is harder than going up. Going does have problems.

Hi doctor my son does circumduction rt leg due to spasticity. Also he bends keen out while circumducting. Which type of brace is advisable.
Another reason...a great toe that can't move up enough to pushoff. Even working to correct the leg can't solve that problem. Botched bunion surgery (twice) and doctors with no clue about needing therapy post-op have ruined me for 50+ years and soft rocker shoes don't cut it.
I have stroke 3 years ago. I have my foot claw when i lift my leg. Can you help to reduce this
You mention floppy foot and bracing but as you have mentioned spastic foot point is not flop foot. I am conflicted because braving both afo and knee cage seem to increase stiff stilt leg tendency. Although safer I compensate with both circumduction and scissor gait.
generally speaking, the combo of a swedish knee cage and afo is the best option for a "stiff leg". Again, generally speaking, an AFO that is set in slight dorsiflexion can also be beneficial/necessary if someone has full ankle dorsiflexion range of motion. I hope that makes sense.

I have a lot of problems to land on the heel. I have ever been a person, who landed on the midfoot, never on the heel, even with my unaffected foot. Isn't it better to not land on the heel, as this causes stress on the joints?
I have a progressive degenerative disorder called Hereditary Spastic Paraplegia which causes spasticity in the hamstrings and calves. I also have weaker quads and dorsiflexors. I drag my toes on forward gait swing, but on back gait swing my heel hits the ground and then my foot slaps the ground. Does that sound like I have both spasticity and foot drop or is that just my weak dorsiflexors talking? I’m still mobile, but I prefer a FES for those dorsiflexors instead of AFO which I feel keeps the ankle at a 90 degree angle and doesn’t train or help the foot to dorsiflex. I have heard that AFO’s will make you weaker and you definitely have to stretch to offset that weakening. I have been thinking about how to exercise to improve coordination of lifting my leg and dorsiflexing on forward swing along with trying to figure out how to control the dropping/slapping of my forefeet.

I've been doing exercises for this.
Toe grips. Feet flat on the floor, curl them under and try to "grip" the floor.
It takes awhile to see results, but it works. I do at least a hundred reps a day.
Till how much time it will be cured and how many sets we have to do ?? Please Mam reply 🙏🙏
