All macrophylla hydrangeas which bloom on old wood should never be pruned in Fall. They always set their buds in Aug-Sept so if any pruning should be done it should only be light pruning right after they are done flowering. Now if they are 5 yrs or older, they can be cut to the ground to rejuvinate and control it's size. Yes, you will sacrifice blooms for that year but it will look fresh and healthy. I had to do this to several of my mopheads. As for the paniculata types, they can be pruned anytime since they always bloom on new wood. So all just depends on what type you have.
