Tah-dah, it's a drug allegory! This isn't the first time Kamen Rider has tackled drugs btw, Kamen Rider W and to a lesser extent Kamen Rider Fourze both have villain gimmicks with addictive side effects.
In the previous episode, Shouma straight up called the twins 'brother' and 'sister'. The one in skirt is the brother and the one in shorts in the sister. Also, in this episode, in the flashback, the eldest brother told Shouma and his mom that their dad was gone. I watched a few different fansubs and some translated it as him being dead. But I'm not sure if that's true.
all the bosses of stomach inc are Shouma's older brothers and sisters, Lango is the boss of everything and the oldest brother, Glotta is the oldest sister and controls how to make the sweets, Nyelv the guy with glasses is the third brother and is also the company scientist, the twins are a little older than shouma and take care of deliveries in the human world, shouma is the youngest brother of all of them
