I don't have siblings so I can't imagine it one way or another, but I think what would make it different than just having two neighbors who grew up together eventually hooking up, is the mentality of it all. With step siblings, you grow up with the mentality that this is your brother or sister, this is your family, and you've been living and sleeping under the same roof since being 1. If I was that guy I would bring up the conversation again. I would need to know the details and if she confirms it whatever "fooling around" meant for her... it's enough for a breakup. Because this guy would be in their lives forever. I couldn't live with that.

This guy needs to leave her as soon as possible. Even for the fact the girlfriend is shoving it in his face being so inappropriate with another male, step bro or not, right in front of him. If they are that touchy feely with each other in front of him imagine how they are when he isn’t there. Secondly, you wouldn’t be okay with any other past lover still hanging out with your gf so this isn’t any different. She’s fully taking advantage of this guy and his gf is walking all over him.

I think I'd be okay with it, bc since they are step brother and sister that means they would not be getting together and he's not a threat to your relationship. At such time as they stop being step brother and sister, then I would ask them to refrain from physical intimacy. I don't think it's right to say your gf can't be close with her step brother after the lifetime they've spent together and everything they've been through. It's totally understandable.
