It depends on your hypotheses. I think you should use model 4 and 1 for mediation and moderation respectively. Use these separately for each hypothesis.

I think the data is automatically being reduced based on the repeat data set. You can use scale data to be on the safe side.

You cannot use control variable in Hayes's process. Just check the mediations/moderations without the control variables.
