Only a few queens (or other forms of female monarchs) have this privilege (Spain, belgium, Luxembourg, Monaco)

The privilege of white is a privilege granted by the Vatican, granted to certain Catholic queens and consorts of Catholic monarchs, (Spain, Belgium, Luxembourg, Monaco) thanks to which they can wear white in an audience with the Pope, being mandatory for the other women who wear black.
In recent years it seems like Pope Francis has relaxed the dress code a bit, which is unfortunate because these kinds of traditions start to lose their meaning.
She is not bowing to him but to the One he is representing on earth. The pope is Vicarius Christi. That's the humility of the king that recognize they can only rule in name of Christ.
