  • In this video I talk about How to Perform a Lumbar Spine Clearing Evaluation. I discuss my pattern of evaluation, my process and why I do it the way I do. This works great for me and may not work great for you. All I ask is that you follow a PATTERN or PROCESS that works for you. Consistency will help you not miss anything.

THANK YOU!!! YOU so helped me since CHIRO said MRI REPORT DIDN'T SHOW S1 irritation, I had researched S1 complications requested doctor order MRI cervical, lumber since both and especially left side calf spasms/cramps all through out night and pulses like a wave in the day, like it's struggling to breathe, he thought it was due to my venous insufficiency. I did all vein pooling test, Doppler test it showed and 3 vascular surgeons agreed nothing to do with unsuccessful vein surgery. I asked for now arterial vein imaging. 15 years ago L5 S1 herniated disc, 2016 again and worked on curvature in low spine.
I JUST did single calf raises as you instructed and cant get up on left side!!!! Still don't know why inner left thigh seizes, spasms with tingles have to pound it and drink pitcher of water but feeling confident it IS a nerve thing!!! Where are you located Paul?

Love your videos, you should also add a video of exercises they should do on top of the methods you'd use to confirm the problem, thanks.

Love these videos. Also when you said pain when coughing or sneezing... THAT IS ME 100%. Quick question had L4/L5 disc rupture, had to have surgery, discectomy, was fine couple of years, re-ruptured same nerve symptoms even worse, couldn't walk stand sleep etc. Had L4/L5 fusion in April 2020, my back is still hurting on the belt line at spine and on left side. 6 months post op doctor thinks maybe disc issue with L5/S1. I think its been bad since day one but they never wanted to address it because the leg pain was from L4/L5. Anyway what are the best exercises I can do to maybe fix it. Coughing kills me and sneezing for 2 seconds then it stops. Rolling in bed kills, also getting up from a chair kills but once I am up pain gone. And my back pain is always worse later in the day toward 9pm. Laying down no pain at all. I can't do anything physical at all. Try to wash my car... kills all day after. Try to cut grass no chance... tried to job 20 steps... pain every time my left foot lands... still wondering is it hardware from fusion or new disc. My doctors keep giving me the run around. Just wait more time.. but this has been going on since week 2. I was off all pain meds 3 days after surgery and the pain was nothing but at 6 months not even close to where i was 5 years ago athletically. I wish he had done fusion of L4/L5/S1 I think I would be 90% right now. Dr.'s assistance said I wouldn't do fusion on L5/S1? Wondering why he said that? I know always better to try everything first but why not fixt the issue for me. Feel like I am 80 instead of 50. Thanks for any exercises that might help the L5/S1.

When checking knee extension from seated vs SLR in supine, what do you mean by “checking for system magnification?” Like assessing for Waddell sign/malingering? Seated slump should produce the same amount of symptoms as SLR test from supine, right?

I thought L3 was knee extension and L2 was hip flexion?

What are all these words 'cheat' and tests I hear from physiotherapists? I can touch the floor with my palms when standing I can raise legs when laying down. I have constant pain from circumferential disc of many years and ligamentum hypertrophy stenosis , nerve damage & meralgia paresthetica. Also severe osteoporosis. I've had numerous falls and weakness in left foot. Two left foot breaks. As I said constant pain and a limp when I walk. What disturbs me is that I can have either leg raised without raising the roof, as you mention. Does this mean I'm cheating NO it does not the MRI shows I have these issues and the constant pain does too. Doesn't mean I can't raise my legs. What stupid tests you have and what worries me is these tests are done to see if people are cheating (your words) Unbelievable really and makes me wonder what physio is for.
