    This reminds me of a lung problem like a pulmonary embolism, caused by a blood clot blocking an artery
    Chest pain does not necessarily indicate an underlying illness as your hospital tests suggest.
    It can be caused by heavy handling, lifting weights, trauma to the chest or swallowing a large piece of food etc.
    I have exactly the same symptoms after receiving a key to the head (strangulation) I have a blood clot behind the neck in the hollow on the left
    and every time I do intense physical activity, lift a heavy load, I have these throbbing chest spasms like a painful tendon of a muscle
    not heated with a panting breath of hot air in my lungs, and the slightest dizziness head turned or lower than its axis, forced to stare at the horizon
    unless you have fibrosis or pulmonary stenosis, which is similar to your case. but it is possible in rare episodic cases
    space out over time until it is just a simple anxiety attack, which would explain the nosebleed from high blood pressure
    continue to have a watchful eye, at best it was nothing and in the worst case it is a benign illness discovered and treated in time

    Here is my best guess. I went through a similar ordeal for many years dealing with both back and chest pain..ribs/sternum. I fixed myself and others with the information I am about to share. The solution might seem dumb...but I will bet you are willing to try and commit to anything that might work at this point. To put it simply your back pain is causing your rib pain and the overall issue there has to do with an imbalance in the strength of the muscles in your back. Because of whatever the issue is with your back, your muscles are not pushing an pulling like they are supposed to. Your muscles in your back are attached to your ribs. This issue with your back is then causing improper movement or lack there of in the area of your ribs causing inflammation known as costochondritis. If I had to guess this is how your issue started...Back pain, Rib pain, chronic pain, depression/anxiety. I don't believe you have some underlying unfound illness. Your bloody nose is likely caused by anxiety. The blacking out could also be a cause of that. Having all of those symptoms coupled with the loss of blood, could certainly lead to a panic attack type state. HERES THE FIX= YOGA. You need to strengthen your muscles in your back. Not the gym, not weights....YOGA. It's free just pull up some youtube videos. HOWEVER....this next part is equally important. Breathing. Since this is a big part of Yoga it shouldnt be an issue but you need to focus on expanding and contracting your lungs deeply. This is part of building that strength. After only about 2 weeks I was completely cured and haven't gone back. I saw doctors for 2 years and had every test and image done..they found nothing. Once you get rid of the pain you can begin to heal the depression since it is mostly linked to your pain.

      I would say I would agree with Kinetic, I had costochondritis in my sternum, and my god, it felt like I was getting stabbed constantly and it got significantly worse with physical exercise and "movement, like deep breathing." However, I was able to pinpoint this because I would go on walks and my symptoms would appear more significantly. I'm not a medical professional, so go talk to your doctor, I know you have already but, I don't think the sharp pain in your rib and the bloody nose are related. One thing that helped me "narrow" it down to an inflammatory response, I took some NSAIDs and went walking and it significantly reduced my pain, I stopped "popping/manipulating my sternum, I would push into it and pop it." If you went to a chiro or maybe even a PT, when you suffer from costochondritis, manipulation of those or movements causes the problem rather than aids it. The crappy part of costochondritis is its the catch-all after they can't see an evident issue. Don't get these mixed up, stretching like yoga and popping or harder manipulation are not the same. Be careful if you take some of those NSAIDs and if you have issues with bleeding, you could just keep bleeding if it is a blood thinner like aspirin. If you know you had blood go down your throat and maybe a significant amount, you may have melena later, so don't start thinking you have a bowel issue at that point. If you're worried, as was I when I had very odd symptoms, check your nasal passageways, you can have an ENT look up your nose or even get an MRI of your brain that will also look at those nasal passageways. As a guy in their 30s, it's totally normal to be thinking of death or health turning and you're gonna be gone ASAP. If you get all these types of scanning, I'd say they definitely rule out a lot of potentially serious medical conditions that could cause life-altering consequences. Costochondritis is the cartilage, so look up a figure and see how far the cartilage goes and see if that is where the pain is similar, the lower ribs go a bit back pain is like on the side.
