    I strongly advise doctor- pls keep guide line in each scenario when it is needed. So that we will not be confused .

    The patient is unconscious and given there are no ambulance notes, do we need to triple immobilization first?

    Question...Shouldn't it be 150 ml of 10% dextrose or 75 ml of 20% glucose?

      Dextrose/Glucose
      25% 100ml
      Or
      10% 200ml
      Or
      5% 400ml

      Dear Dr , guideline says= Dextrose 20% 75 ml ; or Dextrose 10% 150ml ; or Dextrose 50% 30 ml ( but risk of extravasation injury)
      Can you pls clear us the confusion.

      @@rameshlamichhane592 that is when you give it over 10 mins. when you give it in 15 minutes, the quantity is 100 and 200ml.

    During step 'C', after inserting IV cannula and mentioning relevant investigations, can we request the examiner blood glucose level as well? Or do we have to wait till step 'D' for it?

      Yes you can say but BM has nothing to do with bloods it is done by a prick method

      @@DoctorsJourneytoUK Thanks for your reply! Makes sense now. I missed such a silly point. Glucometer comes in step 'D', hence it is more appropriate to request blood glucose level in that step only.

      Offcourse for initial 6 -12 hours. Don't send home straight away. Involve medics and they'll do rest of the things

    If the Oxygen level and RR is normal,why do we still have to give high flow oxygen in hypoglycemia?

      You have repeat Dextrose 3 times before moving to Glucagon. You remember it but no need to verbalise just go on with Dextrose but if you have given it 3 times and Glucose is still low then you can go for Glucagon

      In an emergency, if the patient has a decreased level of consciousness caused by hypoglycaemia, intramuscular glucagon can be given by a family member or friend who has been shown how to use it. If glucagon is not effective after 10 minutes, glucose 10% intravenous infusion should be given.

      @@DoctorsJourneytoUK Hypoglycaemia which causes unconsciousness is an emergency. Patients who are unconscious, having seizures, or who are very aggressive, should have any intravenous insulin stopped, and be treated initially with glucagon. If glucagon is unsuitable, or there is no response after 10 minutes, glucose 10% intravenous infusion, or alternatively glucose 20% intravenous infusion should be given.

      You are right.
      Keep this in your mind please
      Pt conscious = Start Oral Gel then Dextrose .
      2- Pt Unconscious : A- Have I.V. Access = Start Dextrose.
      B- No I.V. Access = Start Glucagon 1mg I.M then Dextrose.

    this approach is different ,and also in real exam patient become confused after administering of glucose , he will not be alert .we need to check the GCS of the patient if unresponsive also that i did my exam

    U just mentioned head tilt, chin lift , jaw thurst look feel listen chest , then move to the moniter .afterwards move to A than again B ..can u plz confirm do we need to access breathing two times here in this station ?

      Initially head tilt, chin lift jaw thrust, listen and look.
      Do A
      Then B including Inspection, Palpation, percussion and auscultation
