Baal is currently under construction. It’s rebuilding the damage from the hive fleet along with building to be the hub of the imperium on that side of the rift so while the hive fleet devastated the planet, it is recovering slowly but it’s surviving.
The fleet size being weird reminds me of Halo 2s opening, with Hood saying "the fleet that destroyed Reach was 50 times this size", which when he said it was wrong because Fall of Reach said it was about 100 ships, but because he said it they retconned it to be like, 500 instead
Aside from the fantastical reasons, leaving the galactic plane would require an enormous amount of energy. Think of how much is required to get a rocket outside of Earth’s atmosphere and then imagine that on the scale of a galaxy. The Milky Way and other galaxies are shaped the way they are because of their weight in the fabric of space and you would have to overcome that to go “under or over” anything within the galactic plane. The ships of the any faction besides Tyranids may not even be capable of leaving

Prisca is an Imperial Navy Admiral, with no ties to the Sisters other than having a force of them under her command for one battle.

It's a bit like a tunnel network, though some tunnels are sealed or infested. You don't teleport, though you travel faster.
