1. Grab admission paper from random smoker. 2. Head to the clinic entrance. 3. Place admission paper on the desk. 4. Suit up in patient robe outfit. 5. Let the clinic security frisk you. 6. Head to the underground secret passage way to where the orderlies are. 7. Wait for one of the orderlies to leave the room and climb through the window. 8. Sedate the orderly in the chair. 9. Take his clothes, the cell key, and the rehab keycard. 10. Hide his body in the container. 11. Go to Agent Smith's cell and talk to him. 12. Sedate the agent to knock him out. 13. Sabotage the gas. 14. Drop the weight on the guy in green. 15. Poison the hidden drink in the globe. 16. Retrieve your suit. 17. Head to the morgue. 18. Revive Agent Smith when nobody is looking and escape.
There is an alternate way that gets you a suit that gives you access to most of the level without you getting frisked, it goes as follows: 1. Climb up on the pipe that's the furthest to the left from the entrance, make sure that the patrolling guard doesn't see you. 2. Sneak your way on top of the roof to the other side of the clinic. (A nurse might hear you sneaking on the roof for whatever reason) 3. Sneak to the guard watching the cameras, there is going to be a door to the left of the window. 4. Sedate the guard and drag his body to a dumpster just outside the door. 5. Take his clothes. Now you have access to all of the level except the Medical Wing. 6. Grab the keycard on the table and take out the camera tape. You can still go through the tunnel without any suspicion until you get to the stairs that lead to the Medical Wing, but now you can also enter the Wing through a hallway that's locked with 2 keycard doors, which is much faster.
Something really weird happened to me in the very end on Professional, where I revived Agent Smith but an orderly thought it was a fantastic idea to come inside the morgue and watch me when I didn't realize. He just stood there and stared though. I was bummed out but thought 'Bugger it, I'll just escape, I hate Flatline anyway and I'll get SA somewhere else' but surprisingly, I still got the SA! Either it bugged out a little or the orderly won't care, just the police. Not gonna complain, though!
for some reason i can never time it to match your time and always have to wait around and figure it out on my own, but these vids definitely help show the way

Noise refers to NPCs in the world hearing your actions. Things like gunshots, attacking people unarmed/anything not fiberwire and syringes, will cause your noise level to go up.
Hey, nice video, i'm having a problem with this mission. I have followed your guide as well as other slightly different ones (all of which got SA at the end) but whenever I get to the end I always only get Professional, the reason being Noise. What am I doing wrong? I thought running around but you do that more than I do and get SA. I'm confused. Any help appreciated.
@stukovwolffire That's a good way to do it, but you pretty much need orderly clothes to talk to Smith. This way you only have to knock out 1 person, unless I just didn't find orderly clothes lying around somewhere.
I did everything right but poisoning the guy's drink at the end. Unfortunately counts as a kill and not an accident, probably discounted my rating from SA to Pro. Don't know why though, it's a lot cleaner than a chandelier.
I don't understand how the game can rate your mission "Silent Assassin" with all the noise you made... But still, good job. What would happen if I just poison the last guy's booze stash? Do I get witnesses?
