FYI, you mixed up MLS cup and Supporters shield. MLS cup is the league trophy but it is won through playoffs by the top 9 teams in the conference. Supporters Shield is really just a bonus trophy given to whoever scores the most points during the regular league season. American sports are all about parity which is why there is a salary cap as well as playoffs between the best teams to actually win the league. It's incredibly different from how football/soccer is played in the rest of the world. In order to get into Concacaf, you have to be one of the 5 teams that win the either Supporters Shield MLS cup, lead either of the conference tables or be the next best team in each conference that didn't win the conference title, Supporters Shield, or MLS Cup until they each a total of 5 teams with the league points record being the tie breaker down the table when one teams holds multiple qualifications spots (such as being second in a conference but winning cup).
To clarify, this basically means the top 2 teams in each conference qualify for champions league every year and any of the other 14 teams can qualify by winning MLS cup or being 5th in the league in points if one of the the other 4 teams that qualify win MLS cup as one of those 4 would have already won supporters Shield.
