i would say it was a screw up, but since it didn't hurt anything, they didn't bother punching in the missing note -- if they noticed it at all.
It was most likely on purpose to give the intro a heavier sound
if it was a screw up there would be a crunchy noise
It was played on purpose, it really accentuates the word "Death"
I thought the instrument that gave structure to this song was the guitar lol
