    Just a suggestion but you should 100% go into announcing at some point in your career. With ultra running gaining so much popularity (and only gaining by the day) you could crush it in the live announcing game. You could give such amazing insight into what the runners are feeling, what the terrain is like etc. You are such a great speaker and would be a natural in that arena (in my opinion).     You are honestly my favorite runner and UA-camr. You have given so much back to the running community. Whether you run or not, you still have our support. Even with your recent struggles VO2 max is potential but does not predict success. Mine is at 59 right now and I'm 51. That's pretty good but my race results aren't that impressive. Potential doesn't mean much without committed training.

    I remember that 2017 video 👍 I was a 31/32min. 10 000m track runner when I was younger.... after 32 years old performance started to decline and now at almost 47 I'm just please to continue running for fun and enjoyement 👍

      I used to like Sage's videos from 2010 and it did help me run longer and faster for a few years, until I started to get injured.
      At the end of the day, running occasionally is fun, but it isn't good for our health to log so many KMs. It ages you a lot if you keep burning your heart.

    Over a 4 year period that coincided with the second half of my 30s, between running, hiking, and backpacking I covered 8k miles and over 1,000,000 feet of elevation, then an old ankle injury caught up with me. I always knew that I'd eventually have to ease up, but I thought I'd get to decide when it was time. It's hard when, somewhat overnight, you lose your hobbies, interests, and outlets. That clearly hasn't exactly been the case for you, but I trust it's still quite hard , and I'm sorry to hear it.

    I can relate (at a much lower performance level than you). About 15 years ago, I had multiple PEs (the DVT actually broke apart during a marathon; of course I didn't know it was a DVT at the time). I had to come to grips with never running as fast again (though in my case, I went from "middle of the pack" to "further back in the middle of the pack"). Wishing you the best in your continued recovery.

      to be fair I've always had "old" cameras. Like even now I only have an iPhone 12 mini and the camera I shot this video with is over 8 years old (it doesn't even touch 4k!). Heck, even my trusty GoPRo is like a Hero 7 only. The kids these days have some fancy cameras sometimes!

    I've been following you for years before a heart attack 2 years ago which I think was brought on by the 'treatment'. But, I still come back here to draw inspiration and to improve my running. Yes, I'm back and started 6 weeks after coming out of hospital. As runners we strive to being the best we can be and take comfort form it.

    You should do another test outside the hospital setting....would be interesting to see another VO2 Max test now, done at the same place you had your highest VO2max test in Boulder

      I bet his VO2max are far beyond than 43, besides even under 69 as predited, but i think maybe test it as has did before, un 2017, can be more stressful and risky than he desires, because he had serious health troubles recently. Its a wisdom choice i think!

    Sage is 37 now?? I'm not a serious runner but I came across his videos when I wanted to run to improve my basketball conditioning. Helped me run my first half marathon. He was the Hansons back then.

    Don't get hung up on the numbers. Just enjoy your runs. We all get older. Even as you slow, it's still good to challenge yourself. I also run because I love being outside and running makes everything just better.

    Dude speaks facts. Honestly there's so much BS out there and Sage cuts through it. JD running formula gives the example of how 2 athletes with similar running performance (results-wise) can have drastically different V02max's, because it's all about the VDot02 - i.e. what your top speed is at your V02max (running economy combined with your aerobic capacity). So yeah, basically ignore the Garmin and go run!

    I just listened to an interview with Bjørn Dæhlie the other day, known in endurance circles because he measured a 96 VO2Max once. He had the lowest VO2Max on the team when he first joined the junior National cross country skiing team here in Norway. His VO2Max was 69 back then, now in his fifties he is back to the same VO2Max as he was back then.

    I agree with you and that to get an accurate Vo2 max you need to do a test specifically for athletes and ideally the same type of test you did before in 2017 to truly compare the values. The medical tests you did aren't designed specifically to test for Vo2 max I'm pretty certain. They are good for lots of other things obviously. But running with a tube in your throat would be ridiculously difficult. If you could get a test just like the one in 2017 I think it would be helpful.

    Sage, I repeat myself. Go check for pulmonary hypertension (echocardiogram or right heart catheter, BNP blood marker). Tests may have to be done under load (exercise) if you feel perfectly alright at rest and initial tests performed at rest show no signs of pulmonary hypertension.

      hey thanks for this (smart thinking)......I actually did all of this this year. So right heart cath and they put me up on the bike and got me going with it inserted! Then a full on pulmonary angiogram, Vent/Perfusion scan, CT scan and BNP blood marker. There was a serious concern for CTEPH and residual/chronic clots. I'm not totally out of the woods yet, but nothing showed abnormal Pulmonary Hypdertenion under increasing load. Heart checked out fine with multiple echos as well.

      Thanks for the update! Great to hear and a real relief to me. I was really worried. I had CTEPH and it started with similar symptoms as a late complication of a pulmonary embolism. I don’t know what your doctor said but regardless I suggest you go for regular checks. I hope you can figure out what’s going on. The best of luck.

    Great stuff Sage! I’m sure your actual VO2 max is much higher than those hospital lab tests and probably your experience/technique/form/overall conditioning make it so that it hardly even matters in races that your (actual) VO2 max has dipped from like 15 years ago.
    Just a question - did running hard for so many years have anything to do with the pulmonary stuff you’re dealing with, or TOTALLY unrelated?

    I started cycling indoors a lot while recovering from a stress reaction. Initially I found that I could not get my heart rate even close to my threshold never mind my max on the bike, my leg muscles would give out before my cardiovascular system while doing a ramp test on Zwift.

    I have no data other than my comfortable running pace and I have lost a minute per mile post-Covid. I have to use my inhaler now when I run. A year after getting sick it doesn’t seem to be getting any better. I am almost 50.

    Hey Sage, very interesting and thanks for sharing. It would be interesting to know what your smartwatch's prediction of your VO2max is? We don't know exactly what is in the algoritm, but since it uses all the training data of you and other athletes it might be more / as reliable as these lab tests sooner or later.

    57 and never lab tested. Cooper was on spring 2810meters. Garmin says 49VO2max and Cooper test 51. It is okay cause I have two illness which limits my training Colitis Ulcerosa and Coronary artery disease.
    But last week still could do interval 5x800meters average 4:06/km.
    Heart was okay.

    Very interesting.
    To me what's most important is the number of steps per minute, very reliable, 186 easy running, 190 steady, 196-198 5k pace 210 800m pace.
    I don't know my Vo2 max ( I' m much older than you are)

    Me paso lo mismo el año pasado. Yo venia en un rendimiento alto en triatlón, en la bici rodaba a 40 kh en zona 2-3, estaba corriendo en zona tres a ritmo de 3:40 mink. El reloj me maraca a un Vo2 de 63-66. En septiembre me enferme y me tocox qurdarme dos meses en cama, me hiceron dos pruebas de V02max en y me salieron en 40 y 47. Manejaba frecuencia cardicas super altas a rimtos super lentos.

    I started jogging right before my 40th birthday after 23years of upper body strength work and couldn't progress nicely within high mileage or nothing better than 10K road 40:48 at 42yr 7mth thought 14 months ago I shifted to pure speed work and lactic tolerance work and managed some PBs at August 2023 being 44yr 7mth old as

    800m (2:17,04)
    1000m (2:55,97)
    I completely stopped following my racing heart rate and just started to focus to pace hard, only thing I wish to have low rest heart rate which I like to measure, let Vo2max to be whatever...

    Maybe you should smash some of the 200 milers? I reckon with your experience you'd do great at those races and I imagine they're less VO2 Max intensive.

    I’m an older runner (much older). My garmin calculated vo2 max dropped from the high 40’s, 47 - 49 to 45-46 after my first Covid vaccine. I never had my heart or lungs tested, but runs over fourteen miles are nearly impossible now.

    Sage, I've been following you for several years. I love your content and appreciate your insight, moral stance, and honesty. You are one of the people who helped me go vegan as an aspiring athlete that cares about the environment and animals. I (we) would like to know what your opinion is on the correlation between your pulmonary embolism/lung issues after getting the "treatment". There has obviously been a lot of commotion around this and the long term fx that many people have suffered. We cannot help but see an extremely strange coincidence around somebody as healthy as yourself having these series of health events happen post vax. Appreciate some candid talk about this without any defense around those who chose to get it and those who didn't. Thanks for all you do🌱✌🧝‍♂

      He's aware of that as i've seen in some comments. He gave some diplomatical answers on this question. He's still sponsored after all, so he can't talk to openly about this anytime soon. It makes me angry how they took advantage of decent people who just tried to do the right thing and how they exploited their trust.

      Given the timing of my pulmonary embolism symptoms and the shot (2nd dose Pfizer April 2021) it very well could have been a cause of the clots. I have no genetic predispositions (no Factor V or Protein C/S deficiency etc), I hadn't been on a long plane flight, and I didn't even show evidence of a DVT. Also no family history for any PE issues. And no surgeries or major injury. I also have no heart defects. Of course there was also a chance I was exposed to the real COVID virus and it caused the clots too. In any case I'd say it is a pretty unlucky and unfortunate thing to happen to anyone. But the timing of events does make one think. There are a bunch of things that can cause a PE in even young athletes training at high altitude who get dehydrated. But in my particular case I actually wasn't running very hard or very much at the time and was seemingly in pretty good overall health. I got PE symptoms about 2 days after my 2nd dose and it started with sharp back pains and trouble breathing. The pain was quite bad and sudden.....it shifted from my right lung and back to my left lung and back over the course of 1-2 days. My one regret was I didn't go to the ER that weekend (I was worried about the financial cost)....if they caught it early it probably wouldn't have caused nearly as much damage. Of course the initial physician assistants that I saw me told me I had
      'pneumonia" initially and that threw everything off .... By week 4 I was coughing up blood and unable to run at even the slowest pace. On week 5 it literally felt like i was going to have a heart attack so that's when I went to the ER and actually got a correct diagnosis and treatment. Modern medicine with blood thinners saved my life.....also not having cholesterol clogging my heart and being in decent aerobic shape probably helped.

    Yeah, it definitely dropped off, but you're still competitive, and how many people in the world can say that? Like you say, you still have your muscles and bones and memories of how to win, and that counts for a lot.

    I’m an exercise physiologist so I love VO2 max tests haha. Your results are interesting. It looks like at max or perhaps “peak” the test indicated you could only use 3.17 L of O2 and you appeared to be at max HR of only 169. Are you now on a beta blocker? We know VO2 max is VO2 = HR x CO -aVO2 difference. I wonder medically if there are really any limiting factors. 169 vs an age predicted max HR of 184 is a noticeable gap, how high has your HR gone in past tests? I also wonder what limits the actual L of O2 your able to use. If it is indeed lung damage it could be a “supply” issue. You must also have genetically gifted cardiac output, particularly stroke volume to have a VO2 as high as you said before haha. I am definitely just an average athlete with probably an average unexceptional stroke volume. What you need is to compare the L of O2 used vs HR and compare that in past tests. Obviously running economy will drastically impact HR vs O2 used, but, you’ve been running for years you probably have similar economy now vs previous tests. I would love to see that data!

    Another thought i had was that at higher altitude air tends to be drier; is this part of the problem deep in your lungs causing irritation and pain in the damaged areas?

    Hey Sage, even if an individual watch VO2Max result isn't as accurate as a lab test, are the relative results you see over time useful? Did you see a similar drop in those results, even if not precisely from 69 to 43?

    Using a HR strap, connected to your watch. Would that give a more accurate Vo2 max reading. Compared to just the watch alone ?

      Chest strap is claimed to give a more accurate HR than wrist based, so in theory. Garmin won’t update the estimate unless you hit 70% of max hr for 10mins. I’ve also found the value lowered by running hilly routes (trails). It’s important to have an accurate max hr recorded. I’ve also tried the guided threshold test using the Garmin Pro chest strap and a value wasn’t returned because despite increasing pace every few minutes my hr didn’t get close enough to saved max value.
      I use the estimated VO2 max (and race predictor) provided by Garmin as a relative measure used to compare my current and past fitness. It’s definitely precise, but doesn’t matter too much if it’s accurate. Kind of like the body fat percentage provided by their smart scale - the number doesn’t exactly matter but it’s useful in comparing trends and current vs past values.

    that seems like a really steep drop over a short time period. I treadmill tested in college, (grad student doing his Phd work needed volunteers), I was around v02 max of 70. My running bests around that time were 4:22 mile, just under 15 min 3 mile, 31 min 6 mile, 2:33 marathon. (note we were running miles, not meters this was before US intercollegiate system went metric !), Pretty much stopped competing seriously shortly after college and going to work fulltime. At age 50 got back into some training ran 5:30 mile, 18:30 5k. I'm a few years away from 70 now, never treadmill retested but recently got one of those 'smart' watches, it's telling me my V02 max is like 40. Just guessing I might be able to run a 5k now around 25. The longest training run I've been able to handle is only around 8 miles. So the decline has for me has been very gradual, I suspect in your case the drop in v02 max may be temporary, and may come back somewhat over time with continued consistent training.

      To be fair to Sage, I brought this up to him previously and he acknowledged the timing of the shots and the clots as a possibility. I feel bad for him and many others who thought they were doing the right thing. Those mRNA shots are very dangerous and studies from other countries are increasingly showing it. And most people who took it didn’t need it.

      if that's true then how can I be 50+ and run 75 mi per week. and have been doing so for nearly 3 years. Vo2 max is guestimated to be 60-64 by my Fitbit

      @@sean5520 most people weren’t harmed. Depending on the study you read, chances are anywhere fro 1 in 700 to 1 in 2000. But you give several billion shots, you’ll see far too many serious injuries.

    How was your rest HR measured? I can't believe it was above 60 with the running background you have. Mine is below 40 with less experience and trainings and I am 49.

      my resting HR (in the morning) is usually in the 40s. But yeah, right before a vO2MAX test in a hot hospital room all amped up and ready to roll I'm sure my pulse increased to around 60

    In theory a good technician should be able to take those test circumstances into account and estimate your "true" VO2 Max. If there's an embedded bias into the test - it should be similar for everyone who takes it. My "guess" is your VO2 Max is temporarily compromised/reduced from the scar tissue - the engine is "injured"

    I think v02 max is very sport specific. I think the bike alone explains it. My vo2 max is somewhere between 55-60 on the bike, I’m not elite, but quick. Running in like 45 🤣🤷‍♂️

      You can measure your max HR with your watch, chest strap or lab device and get different results. Does that mean you have different HRs at the same instant? Same thing if you use the same device to test your max HR with different exercises. You can get different readings but that does not mean you have multiple max HRs.
      Vo2max, like heart rate, is defined as a biological marker. You can get different *measures* of it, BECAUSE how you perform with your vo2max can be different and those indirect measures will vary.

    With the lung damage you've incurred I wonder/suspect that this is affecting pulmonary gas exchange values more than anything else in the overall results of the VO2max number. Did the tests measure cardiac output, a-vO2 difference etc?

    I was diagnosed with hip bursitis last week. I stopped running for 10 days before that, doing all the cross training instead, swimming, Cycling, basketball. I was exercising everyday, averaging 1000 average active calories per day, I thought I kept myself in shape. But no.
    After the doctor visit, I went back to running, my first couple of easy runs, my pace is like 40-50s per km slower at the same heart rate. Or my heart rate would by 10-15 beats higher at the same pace. And this was easy run. I was shocked.
    But after struggling to run for 5 days, my body suddenly went back to my pre-rest fitness level, maybe slightly better than before.
    The lesson is after you stopped running for a while, you need to be patient with the training and fitness.

    Those shots aren't called the clot shots for nothing, should have taken a hard pass. Also recent Swiss study showed 1 in 35 developed clinical myocarditis. I think you got all of those.

      What they have done to people it's absolutely outrageous and criminal. There are plenty of world class athletes who can't go back to its sport after the shots. So sad....
      There are papers dropping off to the left and right worldwide about underreported and serious side effects. A sourse is Dr Cambel.

    I am not sure how exactly Vo2max is related to performance, my highest ever Vo2max measured in a lab was 66 and at the time my best 5K time was a 17:20 and best marathon time was 2:57, which I am sure is way slower than what you are capable right now.......

      those are great times! It's strongly correlated but of course "Running Economy" can make a huge difference (more of a difference than pure Vo2max) with the actual speed and times one runs.

    Garmin says my VO2 max is 43, but i am about 10 years older than you. I am far more a cyclists 😎. I do running in the fall, got a half coming up on October 🙂. Had a VO2 max test done one time at a lab way back in my mid-30s, I looked up the results watching this video, back then my VO2 max they said was 50.

    I don’t think it’s possible to do 6 minute miles at high intensity for over five minutes , and have a “normal” vo2 max. You’d have to be super-economical. Relying on musculature still requires oxygen. They didn’t measure lactate? What about co2 output? If they are testing lung capacity, shouldn’t that be standard? Also to see RER. Maybe altitude had something to do with it. But Without corresponding lactate and co2, and at normal elevation, I’d call the test incomplete.
