I started watching it again but haven't reached this season yet... a lil thing called the pandemic sort of wrecking that achievement ha.. but this episode sticks with me for a very different reason. This isn't the most important scene. From what I remember Cox takes issue with Elliot's insistence that doctors should be healthy and have "good behaviour". Cox does lots of unhealthy things (his relationships aren't psychologically healthy, look at him and Jordan, he comes from an abusive childhood and knows it and has serious anger management issues, he drinks a lot despite being the son of an alcoholic and uses it as a crutch when a lot of patients die of the rabies contamination... he also tells JD that the last cigarette is the best one suggesting he does or did smoke). I think looking back I used to be quite puritanical and think, yes, we should be healthy all the time and be good examples to patients but there's a scene after this one (I think, from the first watching anyway years ago) where Cox admits to Elliot they can't really be the epitome of health. They give their lives to their work and they're going to have unhealthy vices, it just comes with having such an eschewed approach to work/life balance. I totally see that now.. I have worked lots of 24h shifts where I haven't had time to pee or eat properly and when I'm getting really tired I might steal a cigarette with a nurse outside (it's closer to go outside than the other side of the campus to the kitchen/canteen that's closed at weekends). I've also had patients of all ages die, ones that "shouldn't" have on paper because they were fit, young and healthy but it does happen of course... and all I've wanted to do is just scream and leave. I've gone outside in the middle of the night and really thought about quitting.. of course you learn to pick yourself up and keep going and take victories where you can. Anyway, long and short of it is that we can't be really healthy ALL the time, we do our best, that's all any of us can do and we shouldn't judge anyone for unhealthy choices, none of us are perfect, doctor nurse or patient alike.

It can be hard to define the line between doing your best and taking care of yourself.

There's no one state of "buffed". In fact, it's called "buff" and you can be various levels of buff. He's not huge, and in the scenes where we see his torso, 180 becomes reasonable. He's lean and not huge. Just in good shape and muscular.

Only one problem: unlike her patient, Elliot doesn't have a protein deficiency, and we've seen her eat. A LOT. Conch fritters, anyone? Cox should know that different people metabolize food at different rates, and just because someone else is 6'2 and 180 wouldn't necessarily mean that the person was in the same shape he's in.
Yeah, Elliots point was that she's vegetarian and might not be eating enough protein and that she might not be eating enough for how much she runs, not that her weight is the main problem
yeah, but earlier in the episode, elliot was complaining about how most doctors are hypocrites. Kelso telling a patient that smoking will kill him whilst smoking a pipe at the same time, etc, etc
I don’t know about the character Elliot, but Sarah Chalk looks fantastic. Sure if her breasts were larger or her bottom more pert (NOT WIDE but a little more muscle in the right places) and therefore 5-10 pounds in EXACTLY the right places would help, but then she somehow has a “fat” face despite being incredibly thin... Wow I am judgey!!! Point is how ever much she weighs, she looks great, and while there is always room to improve, she is in the top 1/1000000 good looking women in the world. A LITTLE too thin is better than too fat.
