i suppose the monks and nuns who worked in this cathedral turned to satanism and began to sacrifice humans in the last room, causing the whole location to be damned to purgatory. you know, i'm starting to develop a headcanon that all the last locations were the site of human sacrifice, which ultimately caused the rest of the place to be cursed, and the reason the portal out appears there when the level ends is that you finally cleared the location of evil, thus sending all the souls that were innocent to heaven, and all the guilty to hell.

from what I understand from the game, that world is the same like our world, all the locations are real world locations. Its just purgatory a dual world like ours, paralel reality where the dead people are

It took me awhile to notice that the main part of this level appears in the intro. This level is also listed as the first level in game's files and editor :o

this is also the only level in the entire game that has enemies spawned already as soon as you load in. the only level that comes close is the Palace (Oriental Castle), which seems to have a trigger for enemies right under your feet when you start
