The real mistake was that the Jedi never detected the dark influence of Jar Jar Binks.

Yaddle the female yoda was on the council. Got killed by Dooku but he didn't find out until a vision on degobah.
The biggest mistake was whoever let Mace Windu be a master/put on the council. He hasn't yet mastered his emotions.

One thing I can say for certain, George Lucas failed us by selling SW to Disney.

Concidering Yoda's position as Grand Master and being almost a millenia old, he is inevitably responsible for whatever the Jedi Order is. No one had more influence on the Jedi or pretty much the whole Republic than Yoda had and i´dont think he respected the long term, slow impact his flaws might have instilled in the Jedi over generations.
The Jedi Order is what Yoda made it to be, for good or bad. Any major failing it had was his direct fault.
