    Is it Low Back OR the Sacroiliac Joint?
    The patient actually has a Sacroiliac dysfunction/subluxation which mimics 'low back pain'. The referral pain is due to compression of the lumbar and sacral nerve roots. She has hypertonic lumbar erectors, Quadratus Lumborum, and Gluteus Medius muscle, and Piriformis muscle.
    You may experience sacroiliac (SI) joint pain as a sharp, stabbing pain that radiates from your hips and pelvis, up to the lower back, and down to the thighs. Sometimes it may feel numb or tingly, or as if your legs are about to buckle
    The sacroiliac (SI) joint is located in the pelvis. It links the iliac bone (pelvis) to the sacrum (lowest part of the spine above the tailbone). This joint transfers weight and forces between your upper body and legs. It is an essential component for energy transfer between the legs and the torso.
    The Si joint is frequently overlooked as an important cause of low back pain in about 22% of the cases. It is present in about 40% of all low back pain patients that had spinal fusion.
    The SI joint transfers the force and the load from the spine to the legs. The Si joint has very little movement; you don’t want a lot of movement.
    SI joint is protected by strong ligaments that prevent shear forces, and it is surrounded by strong muscles for stabilization during movement.
    The SI joint, the lumbar spine, and the hip share the same pain.
    The pain may be associated with one of these areas or all of these areas. In fact, more than one source of pain may exist.
    Usually, the idiopathic SI joint dysfunction will not show up on the x-ray, MRI, or CT. In fact, up to 25% of asymptomatic patients over 50 years old will have abnormal SI joints on x-rays.
    Causes of SI joint pain?
    The important risk factor is previous lumbar fusion, especially when there is more than three levels involved, similar to adjacent segment disease. So if the spine is fused, then maybe the SI joint will move more and become painful. Pregnancy can cause SI joint pain. Previous trauma to the pelvis can cause SI joint pain. 80% of patients have a history of a specific trauma, usually a twisting injury.

    my lower back issues started exactly at the age of 25. NEVER had back issues before that! then groin injuries followed shortly after. first the right which took a year to heal then the left the tore the very next year smh. also a year to heal. I've had issues with my hips and groin ever since and that was over 24-25 yrs ago.

    Is it normal to have a rhomboid shape depression on the SI joint area, that is slightest darker than the rest of the skin? It hurts right there under that depression.

    Dr. Jawad, is it true that in order to benefit from vitamin D, it must be taken with magnesium?

      It is NOT a 'must' but ---
