The reason why rush can spawn at seek crescendo, because LSPLASH changed some entities behavior, like rush. So this means that rush can spawn at seek crescendo at anytime.

Many modifiers are good, and if they make the game easier, they reduce the amount of door knobs you get after game is over. But there's a lot of them that make the game harder, and they increase the amount of door knobs. And to get modifiers you need to complete the game once. It's not complicated man, you can find a video on youtube called something like "Every modifier in Doors" and it will show you all 60+ modifiers which were all added not so recently.

That's just telling you that you can't use modifiers in rooms. It only spawns when you have modifiers on.
You can't enter The Rooms if you play with modifier
It depends on which modifiers you use, if you only use modifiers that don’t disable achievements you can get it.
