As a rly family man, sanjayji analysis is correct. This is also failure of manual signalling done at the last minute change.
Lakhs of seat are vacant in railway. No recruitment of engineer, staffs and worker . Last RRB vacancy came before 2019 election, Group D joining still pending. What the railway minister are expecting? Lack of manpower and overload on staff lead to this and many incident. They are responsible.

Secondly, search for beer bottles around the signal houses
What is meant by beer bottles? These days there're no "signal house" after interlocking block based systems are installed on all major routes. All block panels are installed in stations only.

This seems to be station master and pointsman, who changes tracks failure. Kavach must be installed on every train length route on a war pace.
Kavach system works only when both the train are on same track n coming towards each other. In this case both express trains were on different tracks
Kavach system bahut costly hei aur bharat jaise desh me ye system baithane ke liye time lagta hei.
Kavach wouldn’t save it these old trains Do not have advanced breaking system because these are some 20-30 year old trains

One of my friends send me an audio clip according to which the home signal for main line was green but the point was set for loop line, some S&T work was going on.
I think S&T staff were working without Disconnection. So SM and S&t staff are responsible for this catastrophe

I used to travel a lot on Coromandel Express during my childhood. In those days air travel was not so widely undertaken as it is done today. It was considered a really prestigious train, though in due course it became slower with more stoppages. Really sad.
