Frozen Embryo Transfer or Fresh - Which is Better? | Infertility TV

  • Frozen embryo transfer success has peaked in recent years. Does it now make sense to skip a fresh transfer completely?
    Hello this is Dr. Randy Morris. I am a Board Certified Fertility Expert and the Medical Director at the world renowned IVF1 fertility center in Naperville, Illinois. I'm here with the second of a few special editions of Infertility TV.
    Recently the international infertility conference known as ESHRE wrapped up in Helsinki Finland. ESHRE is the European Society of Human Reproduction and Embryology. Meetings such as this one allow physicians and scientists to present their latest research to their peers.
    These meetings can serve as an early indicator of which directions fertility treatment may go in the future, what new technologies may become valid treatments and which ones were not able to withstand the rigors of scientific scrutiny.
    In today’s special ESHRE Episode, we are going to talk about freezing embryos

    The basic question is this - If you are doing IVF - Is it better to put the embryos into the uterus right away? Which is known as a “fresh” transfer OR is it better to freeze all the embryos and put them into the uterus at a later time? - which is known as a deferred or frozen transfer
    In the past, the answer was to do a fresh transfer since pregnancy rates on frozen transfers were usually a bit lower. However, over the years freezing technology has been improved.
    At ESHRE this summer Dr Karen Hunter Cohn presented a study in which she examined more than 16,000 IVF treatments performed at 12 fertility centers.
    After matching for different variables, their analysis showed that in certain groups - freezing all of the embryos and having a deferred transfer produced more pregnancies than a fresh transfer.
    Among the groups that showed improvement were
    Women over 35 years old -- 46% of these couples became pregnant using deferred transfer compared to 33% who became pregnant with a fresh transfer.
    There was also a benefit found in patients with elevated levels of the hormone progesterone - prior to egg retrieval - and this was evident whether they were younger or older.
    Because of this and other studies, fertility experts are now debating whether other patients may benefit from the “freeze all approach”.

    I think it depends upon the circumstances at that time. But the frozen embryo is a more prior option than others. Both the methods are helping many infertile couples to bring their happiness back. IVF has an immense contribution behind the success of many infertile couples.

    Well, I have known that frozen embryos are better. But as you have mentioned. Both have their pros and cons. I had frozen embryos. It helped me in avoiding many problems. That totally depends on you which one you prefer. This video was great.

      Hey there. How was your experience? I do not know of anyone who has done this. I think it is an ideal option. SPecifically for women who are postponing having children. It is a safe option. It can help them conceive later.

    Nowadays frozen embryos are preferred over the fresh one. It is because they are much more potent than the fresh one. Fresh embryo leads to excessive production of ovaries and causes many problems in conceiving. Your video has compared both in very good manner.

    As long as they are getting the job done there is really no debate on which is better and which is worse. If it works it is fine. Infertility is very hard and such procedures are very helpful in face of it.

      yes you are right. all the processes the pains and medications are tough but worth the end result. when i got to hold my baby for the first time in my arms believe me i forgot all the pains i had to get him. amazing experience i would say.

    It is really an informative piece to watch. People are often confused about what to do and how to do? The choice of embryos stops them from taking the right decision. It is but not a matter of confusion. Both of them are scientific procedures. Chances of success and failure both exist. Things should be accepted realistically.

    Hello there. You explained it all wonderfully. Frozen embryos do work amazingly. My sister had a similar kind of experience and she is now one happy mom. I love how evolved the medical sciences have become and now being childless is no more an option. Good work there.

    I am so happy to see such kind of treatments of infertility. In this modern era, we have a solution to every problem. In the past, people have to face lots of problems to fight against such diseases. My two anties are still passing their life without children because they are unknown with such treatment.     Thank you for answering this most essential question. This is a cause of confusion for many couples. IVF is all about making the right decision. I personally think it really depends on your body and the condition. According to which you should decide about the transfer. I will soon be starting my process as well. It is very lengthy and I am really nervous. I hope everything goes fine.

    Thanks a lot for sharing this video. This has helped raise awareness on the topic of frozen embryos. This is the topic that s rarely talked about. I am glad that there are many people who are taking a stand on sharing information regarding topics that were initially never discussed openly. I am also super excited now to make a chief for myself. This is extremely helpful for the couples who are still confused about their stake in these treatments. But thanks to the technology that has made our lives easier. And thus many couples are now able to conceive and experience parenthood.

    FET is the great advancement in medical science. IVF is a safe and effective treatment for infertility. IVF has very low risks to the patient. The risks include ovarian hyperstimulation syndrome, multiple gestations, ovarian torsion, ectopic pregnancy, and risks of the egg harvest procedure. Anyways, I wish It will be safe in the future.

    Indeed frozen embryo transfer is getting common and best treatment to deal with infertility .but there is a lot of confusion between fresh transfer and freeze embryo. I was confused between these two. I was thinking that both are same. well explained Dr.Randy Morris.after watching this video all my doubts have cleared.thank you for making us aware about it.     Hey Doctor, you spoke very well. It is believed that because frozen embryos would be used long after the egg stimulation hormone therapy is done, the woman’s body would have returned to a natural state hormonally speaking.

I was declared with infertility 6 years ago and opted for IVF. Although I chose fresh transfer, I think both of them are amazing. It is good to see so many people who are infertile going for this.

    Well, it depends. I mean I have my own opinion. I was aging too fast. I had to get them frozen. It worked for me. I went for an IVF. Used my frozen embryos. Now I have a daughter.

    Technology s helping mankind in many ways. Embryo transfer is one of them. Th technology helping many couples to have babies. My friend had gone through the same phase. She is satisfied with this treatment. She contacted a good clinic for the transfer. Thankfully her treatment was successful. She is now mother of a baby.

    Thank you for your reply. Can you give me any idea why my fresh transfer wasn't successful. I'm 38 I had icsi and we had a AA hatching blastocyst and a text book womb. They said it wasn't cesrrating the right chemicals? I have a AB blastocyst on ice. But I really don't understand and I'm thinking is this just a game of chance?

    I had my IVF 1st transfer at exactly 15 days I bled. The doctor gave me higher dose of progesterone then after 1 week i visited my original doctor and said I had to stop all the medication because the embryos already dead. After i stopped medications I bleed and the big blood with a tail came out. Then I had my second attemp from the same frozen group of embryo. At the day of the transfer at least 4 hours before the embryo transfer the clinic immediately informed me that the embryo died during thawing. The problem here is that we have fully paid the program and they already closed this programmed as consumed. I am protesting that the egg was not a good egg because nothing had been survived and the IVF clinic wants us to pay again if we wanted to continue the program. The full payment that we did is already voided according to them. This is horrible! The embryo died during thawing so either the it is already damaged before or after freezing and it is not my fault but they want to take all the money that we had paid for that! Is that possible? Is that correct? That the clinic donot like to answer for the failed embryo or take our money as easy as that?

      Embryo freezing is not without risk. Freezing of embryos can cause egg crystals to form in the embryo. Upon thawing, these ice crystals can damage the embryo. At IVF1, our survival rate upon thawing embryos is around 97-98%. These survival rates can vary widely according to the techniques that a program uses.

    @InfertilityTV - Hi, you mentioned certain groups like women above 35. What about low responders in their 40's doing blastocyst transfers. Is it also better to do frozen compared to fresh? I'm confused because the Duke study says something else regarding low responders. Thank you for an answer.

    Hi. Iv just had fresh transfer fail. I think I had ohss at the time could this be the reason it failed. I had a AA blastocyst and text book bomb I'm 38 years old and I'm waiting for a FET in three weeks with my frozen blastocyst. I think the clinic should check for ohss. Why waste a good embryo

      OHSS is more common and more severe in pregnant patients. There is no evidence that it is associated with lower pregnancy rates.

    Does embryo transfer technique cause ectopic in ivf?what is the mechanism that embryos migrate into the tube from uterus? Usually embryo formed in fallopian tube migrates to uterus in normal conception with the help of cilia beating towards uterus in fallopian tube.

      There is limited evidence that transfer too high in the uterine cavity may increase the risk for an ectopic. The fallopian tube does a number of things: clilia, contractions, secretions, electrical impulses etc that when WORKING correctly should allow sperm up and a fertilized egg down. It is theorized that abnormalities in these functions are what causes an ectopic

    Hello Dr randy I have a question on you. I am 21 years old now and live in austria. I was diagnosed with an uterus septum and Endometrioses Stage 2. This all was removed in June 2017. In August 2017 I had my ivf where all embryos where frozen. In October I had the first transfer of an blastozyst and I'd worked but at least it was a biochemical pregnancy. I have now 13 blastocyst frozen. Do you think I have a good change to get pregnant with this? Can you make a video where you were talking about some extras what a person can do by a Fet day 5 transfer? I mean embryo glue, assisted hatching etc. Yours Michelle
