When to worry about speech delay in toddlers? - Dr. Satish Babu K

- A perceived delay in speech or a baby failing to speak like peers of his or her own age is definitely a concern for the parents. So we should know when to worry about it and when to take consultation with the doctor, therapist for that matter to sort out the problem at the earliest. Let us look at the development of the speech in a child. New born baby starts hearing from the moment it is born, though it doesn’t make any sense to the baby, the sounds keep falling to the ear and it reaches the brain slowly as the brain matures, it starts deciphering the sounds. So it starts making sense of the sounds. So the baby starts understanding what is being said or what is being communicated. Once that happens, the baby responds to the sound stimulus. So first 3- 4 months, the baby takes time in grasping the sound. After that it starts imitating. Speech develops by imitating the parents and other relatives around who keep vocalizing and speaking to the baby. First for that the baby has to hear the sound and understand it and reproduce it sensibly. The first 4 -6 months of age, the baby starts producing cooing or babbling sounds, atleast starts vocalizing. That means the baby is able to recognize sounds and reproduce it. It can even recognize the mother’s voice or the voice that is frequently spoken to the baby. By 6 - 9months the baby is able to grasp better sounds and probably you know produce sounds like mamma or baba, you know gently, it also tends to follow single or simple commands, like sit down, and stand up and come here, these kind of commands the baby can understand and follow. That means the baby is grasping sounds and making sense of it. By 9 months, 9 - 12 months we can say, that the baby is able to understand complicated sentences or two stage sentences and then produce one or two words, not as a sentence, but try to make or coin a sentence in that process. By 5months of age, the baby should be able to make simple sentences and should be able to understand complicated words or complicated sentences to follow the orders. If by 2 years of age, the baby is still not able to make these kind of atleast 2 word sentences or sensible speech, then it is time to worry about it and get to the doctor’s chamber as soon as possible. What are the warning signs of a possible problem? First and foremost, we should look at the baby’s ability to hear. If the baby is able to hear like his peers, atleast comparable to his own age children nearby or his siblings, then there should be a problem there. If the baby goes too close to the TV for example that means he or she is not hearing well, that he needs to go close to the TV to hear better. So that is one of the warning signs that is commonly seen. If the baby is inattentive, trying to call the baby for some reason and the baby is not responding to your calls consistently, not because your baby is concentrating somewhere else, not listening to your voice, even when the baby is not very busy, still he or she is not able to listen to your voice, then there is some problem. When the baby is not hearing well, obviously his speech will not be also developing very well. The other pointer we should look for is the baby starts using gestures rather than words using to communicate like for example the baby wants water, the baby just signs with the thumb towards the mouth, then it means that the baby has some problem in communicating. We should try to rectify that as early as possible. A baby which watches intensely whatever is happening around him or her but not responding to voice stimulus or sound stimulus again should be checked for any problems for hearing. A baby who doesn’t do a reflex action or doesn’t start crying when there is a loud noise around him like a door shut or a window pane falling should create a reaction, a fear reaction in the baby immediately. If that doesn’t happen or a cracker bursting nearby and the baby is not turning towards it, then there I some problem with the hearing. There are other issues which can lead to problem inspite of the baby hearing better, like a tongue tie is most common of that where if the baby is not able to lift the tongue above the floor of the mouth, we should look for that and get it corrected at the earliest so that the speech can develop comfortably. The most important thing that the parent can do is to keep talking to the baby right from birth. So the baby is getting used to the sound stimulus and starts making, and the brain also gets tuned to the sound stimulus and you know, the baby can develop speech at the earliest and then do not encourage gesture commands or gesture communications and demand the baby to speak atleast one word, like if the baby wants water, the baby should be able to tell atleast “water” clearly to get the water for him to drink. If we encourage gestures and keep providing the work, then obviously he doesn’t have the need to speak and he will not develop the language fluently.

I had a speech delay as a baby. At roughly age 2 apparently I was mostly saying "mommy" and "milk". Years later I don't shut up.

This is really informative, I know a child who is 2 and can speak very well, complete sentences and I'm just curious why some children 3 + have a hard time with simple phrases

Depends on their exposure to the language
Same problem
Simple, all kids are different and their reaching the milestones depends on their genetics and their environment. Genetics is a reflection of parents mental fitness and how the genes mixed up (which may also mix up such that there is some defect or deformity). Apart from that we have how the child is raised and what stimuli it gets from its environment.
So some kid speaking at 2 simply means that nature and nurture are all set, while another not speaking at 3+ means there may be a spectrum of issues, some curable, some manageable.

I mean she two and half years old but she can't say apple .. she is a intelligent girl .. she observes very well .. Sentence at 15 months? Even Books on paediatric doesn't say that
Sentences like "come here""I want milk" "i peed" . Those are two words sentences. Good grammar just less words.

Hello , I have my daughter who's 3 year old and not talking yet ...... she's use to say word like """ Go " but she stop saying it ..and she said word like ( didi, mama, manman , mom )) but she's respond to her name , when say thing like sit down , come to eat , pickup things, when she wants something she's going to take your hand and point at the things she want , she knows all the colors , she's matching everything. I know she's going to talk in the name of Jesus-Christ I have faith. Anybody please let me know your tought.
My 2 year old nephew doesn’t do any of that. He doesn’t even point towards anything he wants. But he understands his name and comes when called…he doesn’t dance or try to sing-a-long to songs

May be I'm not repeating sound in front of him. He is not clearly picking a sound or a word from me. My son is 2.5 years...He speaks mama pappa nana nanni...but what he wants he speaks in gestures...Like if he wants water he give me glass...He reapond to every thing we say....What are the possible things we should do make him speaks early...And Because of Covid We cant roam him asa much possible like early with kids.!!!

My child started speaking word like papa, mumma, lala at the age of 15 month after age of 20 month he sufferred from fever and he is not pronouncing word that he speaked is there any problem

My son was kidnapped by his father at 8 months old. I know it affected him because he started walking late. And now he won't speak even though he can he just points and holds out his cup towards me for drinks. I wonder if my son needs therapy. He was stuck with his father for over six months lost a sad amount of weight even his spunk that he had before was gone. My child experienced heartbreak at an early age and I don't know what to do.
He just needs lots of love. He has lost trust which is the utmost thing.. just give him hugs without reasons.. cuddle and sleep with him.. go for walks.. amusement park.. shopping .. spend quality time with him.. explore new places.. boost his confidence and the utmost thing pray to almighty.. he is a magician.. pray with ur baby everytime he goes to sleep.. mornings.. this will bring positivity
AT that age the child needs mother way more then a father. That probably left him with a trauma as to where mommy is. As Mrinalini said, give him lots of love and promote him to speak instead of agreeing to his actions. That will give him the urge to say something to get what he wants.

@@themaxtreme69 your absolutely right. I have since learned that it's even bad to do to baby animal and its mom from the holy Bible. The Bible even states that as a law not to do so to animals! I have extra patience for my son because he doesn't talk much yet with the exception of one year old level talking,, but I feel that when he does he will catch up really fast. I can see that he's smart in other areas. For now he still points to every thing but at least he is appropriate in his actions. So that's comforting.

Hi, New here! My 18mos old boy is not talking yet. But he babbles and sounds mama, papa,baba,ne,la,he,ka aahh , but he doesn't say words. But he can understand what we are saying like if we tell him not to go there, or go back here he can understand it. He also shouts when he doesn't like the video he's watching on the tv It's like he's telling us to change the video since he doesn't like it. Is he delayed? Or just developing his language

7 months is way.....too early, they're not gonna start speaking until atleast 18 months, even then there're so many toddlers who don't speak, but that's still ok

Respected sir, my baby 3 years and 4 month and she can speek few words. She has learnt alphabets colur name and whatever I teach. She speak. But she don't communicate with us like can't speak full sentence. I checked up earlier when she was 2 years they didn't check correctly and on the basis of my thoughts they told she has autism behaviour. She needs therapy. Her ear and tongue is fine they check at birth time. Presently we r giving homeopathy medicine from 2 months. Please tell me Is she is autism behaviour or speech delay.
