It’s mainly the idols of East Asia that we see which makes everyone in East Asia seem like they are all skinny. There are people there who are muscular and work out a lot and there are people who prefer the lean and slim body figure. It’s not much different than places like America, it’s just the obesity rate is much lower. There are muscular people and there are skinny people based on their own preference. Not all East Asian boys have time for make-up, it’s just a lot of idols wear make-up for photo shoots, concerts, fansigns, etc. which makes it seem like everyone does. Outside of those events you see them barefaced walking around on the streets. Skincare is a large focus though so many East Asians have either naturally nice skin or use a lot of skincare to achieve that look. Cosmetic surgeries are quite common sadly. East Asians strive for the “natural” make-up look but it does not mean they don’t put on a lot of make-up. Girls do wear a lot of make-up but not many people go to these extremes in the video everyday.

I think it's crazy how they make a round face look heart shaped, and small eyes look huge. It's definitely an art form.
