Also, for the napkin: if you're going to a restroom and are coming back, the napkin goes on the seat of your chair, but when you're finished with the meal and leaving, put the napkin to the left from your plate. I have no idea why this lady doesn't know these things, she should if she's going to talk about them!

Upward pointing tines are a bad eye poke waiting to happen, if you should accidentally push down on the handle of it. To be honest, dining etiquette is not something you can cover in a 5-7 minute segment. There's logic to each rule, and plenty of hands-on practice is required. It should be taught at home, when the student is young and impressionable. I think also, if you're doing continental stay with that. If you're committing to American style, stick with that for the duration of the meal. You want the mechanics of eating to be secondary. Your conversation is primary, after all.
