That's not quite how it works. In actuality, the life counter begins when at least half of the players have given up. That means in 4-player, it starts when there's only 2 players left, and in 3- or 2-player, it starts when there's only 1 player left.

He crammed a bunch of springs into one tile, and the game calculating the collision detection of each spring on each spring is pretty taxing on the game. Something similar was possible in MM1, cramming POW blocks did the same thing in that game.

Waiting in front of the flag is actually doing others a favor, since once the player (Toadette) see's the other player (Toad) she/he will grab the flag, the others can grab it and not lose as many Gamer Points.
