I'm surprised nobody has commented that this theme is actually a jacked-up version of the normal boss music. Which makes sense, given you fight the Fallen before Satan reveals himself.
This also reminds me of the Cellar theme, which is also a different version of the boss theme. Which makes me think which one is actually the "original" one.
Also if you listen to the background guitar in the beginning it has kind of the same tune as the "Penance" track from vanilla.
All the boss music follows the same general theme.

I love how Hericide and Infanticide contrast. Hericide has more a band vibe to the song especially with the guitar solo while Infanticide has a more orchestral vibe too it yet both have a hymn choir in the background.
Original theme better fits Satan.

contrary to popular belief you can enjoy two things at the same time
FINALLY I was able to beat Mega Satan with the Lost in Rebirth... I just did it 1 hour ago and even though you don't get anything from it, I feel like I've finally fully beaten the game. Getting to Mega Satan is the real problem not the fight itself, since you need the 2 key pieces and with the Lost, you hardly get any Angel rooms. I was lucky enough to get Dad's key in the 2nd floor and turn into Guppy just two floors later.
I actually died twice against Mega Satan but I had Guppy's Collar and was lucky enough to get revived twice, since it only has a 50/50 chance of reviving you. The only thing left now is to get Godhead at least once, never got it yet.
I'm still confused as to why a game where you play as a naked perpetually crying child while navigating your mom's basement has in it the pure blasphemy of a creature called fucking Mega Satan.
I dunno, it has really fun and addicting gameplay with tons of replay value.
Story's kind of fucked up tho, though its supposed to be a commentary on how religion can be taken too far and used as an excuse to cause harm. The creator himself even comes from a religiously abusive household.
Try reading about the game's plot a little, the "story" the game presents upfront isnt what's really going on.
Oh, I'm sorry if my original comment seemed to imply I hadn't played the game before; I've looked into the lore, played the game around a thousand times and it never gets old.
It's a bit less literal than that. I think. I've read other people's takes on it, and added some of my own thoughts. *SPOILER ALERT* From my understanding, Isaac lived with his father, mother and sister (Magdalene). Isaac's mom had a series of miscarriages (or abortions), and was eventually diagnosed with cancer. As a resort, she turned to religion, but had a breakdown and started abusing her children. I assume her father left her, but only managed to take Maggie with him. I like to think that the dad tried to get Isaac away from his mother's zeal, and tried to teach him science (hence the two main enemies in the game are biblical or related to anatomy). On heavy medication (the pills) and constantly listening to Chirstian broadcasts, Isaac's mother hears voices and tries to sacrifice her own remaining son, after abusing him and withdrawing him from school (cutscenes). God interferes, knocking her out with the Bible. Now Isaac has been half hearing the broadcasts for a while now, as well as reading the Bible himself. and is convinced that his mother is right: he is sinful. He locks himself in the toy chest in his room, where he had been building a fantasy world (the actual game) as escapeism. I assume the cutscenes are when he recalls the shit that went on in the real world. He has trouble coping with many issues: his mother's abortions/miscariages, him blaming himself for separating the family (and breaking his mom's heart), etc. He dies in that chest (becoming ???) or is dying there (not intentionally though), and the extreme conditions cause his hallucinations to be more vivid. In this fantasy, the player gets 2 (or 3) choices:
-Descend into Hell, either to punish himself for his perceived sins, or to defeat Satan as a way of cleansing himself. Isaac thinks he's defeated evil and tries to get back into reality... but it's too late. The Dark Room reminded me of lucid nightmares I've had; it feels like an out-of-body experience, where I am still in my room, but it's much darker, sinister, and I can't leave it; for a young Isaac who wasn't able to escape the "chest" of his mind, this is his own personal Limbo. The Lamb is Isaac's interpretation of what he's heard on his mom's TV: the scapegoat of our sins, perhaps his last chance of redemption. TLDR - Chapter 1-Sheol = Isaac's mind, Dark Room and beyond = Isaac's Spirit
-Ascend to Heaven, hopeful about redemption. In the Cathedral, Isaac fights himself to prove his worth. But the important this is the music in the background, the lyrics of which roughly translate to "you should not fear the one remaining God; He will free you of your body; He is waiting". Assuming that God exists in Isaac's world, Isaac is being reassured that it's not too late for Salvation. Confident after beating himself, he tries to leave his mind-game... only to find that his spirit is trapped inside the Chest, where he must face the fact that he died (fighting ???) in order to move on. TLDR - Chapter 1-Cathedral = Isaac's mind, The Chest and beyond = Isaac's Spirit
-After being stuck on a loop, I assume that Isaac realises that he's been manipulated by the actual Satan (Mega Satan), and his spirit is willing to do anything to avenge himself. Again, the soundtrack is important here: the theme while fighting Satan (a figment of his imagination) is the same as when fighting Mega Satan; it is titled "Hericide", which means killing one's own master. Isaac destroys the two angels guarding the key to Mega Satan's room, the same angels that had blessed him with "free" gifts, and fights the end-game boss.
This is as complete a picture as i can make. Sorry for the length lol

+Augusto Daro interesting lore but keep in mind that the story has a small similarity to the binding of Isaac in the Bible when Abraham received the same command for a similar reason but God intervenes just before Isaac is sacrificed and a beast is sacrificed instead. I think that the difference though is that Isaac's mom tried to do it without understanding the significance of the order (or she hallucinated).
Fun Fact: As most of us may of guessed, Hericide, Matracide, Infanticide, and Ventricide have some obscure meaning, But i find Hericide's most interesting. It is a term meaning to kill ones master, Meaning what you may say? Plot twist, Isaac is a not so innocent.
***** I think that instead of having Satan help Isaac, he actually made all this up so Isaac could come to his side. What if he was the voice Mom heard "from above"? Satan is perfectly capable of doing so. Driving Mom to attempt killing her son made Isaac think about all the concepts we see in the game (the game is part of the pictures he drew), some of them being the deals with the devil and the death of his Mom. ***** Well, in the end, Isaac ended up surpassing Satan himself. Watch ending #16.
No, it's herecide as in "death of heresy". all of the words are latin, infant as in baby, matri as in mother. The "cide" at the end means "death of". genocide, killing of a race, pesticide, kills bugs or pests, herbicide, kills plants. Ventri, womb. The meanings are the OPPOSITE of obscure. Killing the heart? Music is "death of womb". Killing mom? "death of mother". "Here" (heh-reh) as in heresy, going against orthodox, opposite of proper religion. Fitting, as Satanism is unholy, the opposite of Christianity. I checked everywhere, doesn't mean master at all. Where did you get that from?

Oh, damn. Yeah, you're right. To the argument that he's corrupt, in the cathedral ending, you see him reading the bible, then seeing himself as evil. In the bible, it states that man is born with sin. Maybe Isaac thought he deserved to die, since god in the bible asked for him to be killed. But in the Mega Satan ending, he actually turns into the devil. But, what I don't get is, why? Why does he become what he fears the most?

The Rebirth soundtrack definitely falls short in some places compared to the original, but this is definitely not one of those places.
There are a few soundtracks I don't like, like: the new depths theme, new shop theme and the new catacombs theme. Every other track, I love.
Both soundtracks are really cool. For example, fighting mom in original feels like gathering energy and here i love this dark chorus in satan fight

I remember the first time I got to mega satan and this blasted right in to my ears and my reaction was "HOOOOOOOOOOOOLY FUCK!!!" That's one hell of an intro to an amazing theme....... No pun intended..

Rebirth doesn't have as memorable a soundtrack as the original, but DAMN! It's way more intense!
It's not memorable because the melody is way harder to remember, but this OST is way better, except for few tracks.
No, Rebirth has far worse music. This one is nice, but tracks like My Innermost Apocalypse, Mom's Theme and the Basement music from the original soundtrack shall never be forgotten or beaten.

I'm really late to the party and I've first properly begun playing this game now (a bit too hooked, of course). I played Flash back in the day religiously. Even though I knew most stuff from the original, all the newly added content brought a pure sense of wonder. Going to the Dark Room for the first time (thinking nothing would happen post-Sheol) and wondering about the huge locked door. Bombing a blue flame in an Angel Room to get a Soul Heart and accidentally triggering the fights to get the Key Pieces. And then actually reaching Mega Satan himself... To say the first time was an adrenaline-filled rush was an understatement. Him unleashing the entire arsenal of the biblically themed foes (The Horsemen, the Deadly Sins, etc). Such a cool fight.
It's also playing during the fight against MEGA SATAN! (I was expecting for another ost D:)

hericide means killing one's lord or master, infanticide means killing a child that is not within a year of birth which is usually someones own child, matricide means killing your own mother, anad ventricide means killing someone by inflicting damage on their heart. so basically all of the major themes in this game are about death.
