Every inverter has switching table in theory. You need to follow the sequence given in table, and before doing this select output voltage frequency so that it would be easy to get precisely select time axis point or delay in pulse generator.

You lost me a 6:18 . You did not explain how the circuit on the left generates the 5 levels on the right . This is the core concept of this video but it is not explained .

About PWM its depend on Frequency of Output voltage, For this dear you will need to go through PWM switching table, and to study that how to get the output voltage in segments using turning on different switches. This theory or methods are available in literature and text book also. You can search in Google that switching table of 5 Level Inverter. Still if you dont find it leave comment i will send you,
About the THD, First thing you will need to decide what harmonics you are wish to remove. Based on this you can design your Inverters, This is Five level inverter hence we know it will reduce 5th Harmonics from the output AC voltage.
