To me, what he's saying is her words are destroying his heart. Don't know if that's the way he meant it, but that's my take.
I interpreted as granite as weight in his chest as in doubt or misgivings about the relationship. If she would actually talk with him, to communicate. By doing so she had the ability to dissolve that granite in his chest. But all their relationship was physical & arguing.

And you can see scars on his arms during live performances or still pics of the band's performances. So it may be an actual event, we may never know. My take is the blockhead refers to the denial. We're closed off from all the red flags and toxicity, we don't want to see it, and don't want to face it. Notice the cape is tattered. It's battle scars. He's been fighting a lot of battles in this toxic relationship. No shield so no defenses. Double axes, all offensive, berserker style. All attack, keep going at it. It fits the songs aggressive tone. The black tar covering him at the end is the darkness of it all just consuming him. The notion of a car crash is in several of his songs. I don't know if it's literal or metaphorical, but either way, something tragic happened that's obviously affected him in big ways. To me, "second hand smoke" refers to him getting the bad effects of her toxicity. She's basking in this toxicity and he's suffering second hand. "The glass on the street" I'm still not sure about that one. "You go half-blind when looking at me." This is not wanting to address the elephant in the room. You're in denial of it. He stays in this viscous and toxic cycle with her because it beats having nothing or nobody, even though he knows it's to his detriment. I always caught the glass on the street and keep your eyes on the road line but for some reason I never connected it to car crash, as soon as you saw that I could picture a more traditional MV, the intro looking up at a streetlamp at night, the flicker of a fire reflected against the metal, a pan across the asphalt with glass scattered around, flickering to brief clips of them in the car, after the hook there could be a pause where it cuts to a scene of them getting in the car after a party or something, she's obviously upset, slamming the door, maybe she took the keys from him to showcase her being controlling. Then it cuts back to the chorus of them speeding down the highway arguing, her banging on the steering wheel, he raises his voice she flinches, its so visibly toxic between them. When he says "keep your eye on the road" the camera could be looking at him as headlights appear in his window behind him, then back to the hook it pans over the glass again. keeps on like that until the metal drop where it cuts to the scene with the lights behind him again during the little pause, and slow motion as it gets bigger and brighter, then at the drop the actual crash, the car rolling about, maybe smaller cuts to him watching as he sings, and as the song ends she comes and holds his hand as they look at themselves in the wreck, the camera zooming out from behind them, their silohettes dark against the fire, before cutting to black. It’s a big ask, but you really should listen to the whole discography in order. It tells a story and there are a TON of fan theories about the car crash because this is a pretty constant theme throughout a lot of songs.
