The third phase of DMAIC is the Analyze phase. The primary objective of the Analyze phase is to identify and verify the root cause of problems inherent in a process.
Excellent overview of the PFMEA and how to create one. Not enough companies use this powerful tool (usually only automotive industries, where is required as part of the PPAP process). Now though the aerospace and other industries are adopting it due to the "RISK ASSESSMENT" section of the new AS9100 Rev D and ISO 9001:2015 versions where FMEAs and Control Plans are critical documents to show you have analyzed your process and company risks and taken actions on them to improve. Also important on the FMEA is not only having an action on the High RPN # issue but also the high severity issue which could lead to Death or Injury or legal issues for the company. Only thing on this video is that the FMEA is NOT the only tool used in Analyze and there are many others like Minitab for Statistical analysis, Pareto, Run/Time Series charts, etc etc. But this is a short into video so its ok but should at least mention the other possible QC tools.
