  • Sickle cell anemia is a genetic blood disorder that occurs when a patient has abnormal hemoglobin on their red blood cell called Hemoglobin S (normal hemoglobin type is A).
Hemoglobin S is very sensitive to low oxygen level. When a red blood cell with hemoglobin S experiences a decrease is oxygen it causes the red blood cell to change it's shape from a round, smooth shape to a stiff, sickle-shaped, and sticky shape.
This change of shape in the red blood cell leads it to occlude circulation within the vessel, which can lead to various types of sickle cell crises (vaso-occlusive, spleen sequestration, hyperhemolyic, and aplastic crisis).
Sickle cell anemia is an autosomal recessive disorder that is homozygous. This means that both parents will each give their offspring one abnormal hemoglobin gene (Hemoglobin S) to make their child have hemoglobin SS. The parents are typically carriers of the disease without symptoms. The parents are heterozygous in that they each have sickle cell trait (in other words they each have hemoglobin AS), but not the actual disease.
African-Americans are most commonly affected with this disease. According to CDC.gov, 1 in 12 African-Americans carry the sickle cell trait. In addition, Middle Easter, Asian, Caribbean, and East Mediterranean patients are at risk as well.
Many patients with sickle cell anemia will have symptoms of anemia, but will not manifest major signs and symptoms until a sickling episode occurs. Factors that can cause a sickle cell crisis include: significant blood loss, illness, climbing or flying to high altitudes, stress, dehydration, fever, strenuous exercise, or extreme cold temperatures.
Sickle cell crises vary among patients with some patients experiencing random episodes with others experience them frequently. Sickle cell anemia is typically discovered in the young pediatric patient (age 6 months and onward). These patients will present with swelling of the hands and feet (hand-foot syndrome or Dactylitis), fever, and be extremely fussy.
Patients who have sickle cell anemia are at risk for the following complications: acute chest syndrome, stroke, splenomegaly, infection, leg ulcers (mainly older children), anemia, vision changes, gallstones etc.
Nursing interventions for sickle cell anemia include: educating the patient or parents about the factors that lead to sickle cell crisis, maintain hydration, administering oxygen, pain control, bedrest, prevent infection and assessing vaccination history of patient, elevating swollen extremities, removing occlusive clothing or objects etc.
A medication used to treat sickle cell anemia is Hydroxyurea, which helps make fetal hemoglobin. This will decrease sickling episodes and the need for blood transfusions. However, a low white blood cell count is a side effect of this medication.

I am a person who has sickle cell anemia who also carries the SS version and having it for years; I want to say that your video has educated me more than the many doctors I have come in contact with. Thank you for making this video! I normally don't get pain within my joints but tend to suffer from the Acute Chest Syndrome, but after listening to this video I'm going to drink a ton of water and stay healthier, especially when working out. So, again Thank you! Even with having the disease Education is always needed!

There is a huge loss of zinc in sickle cell anemia. Foods like millet are a good source, which is why millet is recommended for the diet in sickle cell. Supplementing zinc can cut the number of crises in half. I strongly urge you to explore scientific data and ask your pharmacist and doctor about it.

I’ve been dealing with chest pain for the last 2 months which felt like a heart attack but doctors just think I’m crazy at this point.

This is a very comprehensive and complete video, and very clearly taught. Thank you for teaching this! I am not a nurse, but a parent of two sickle cell children who are now young adults. We have definitely had our challenges, and I know from experience how vitally important it is for the health care provider to have the appropriate expertise in treating sickle cell patients. Thanks, again. (I am currently a graduate student in nutrition and studying research on the gut microbiome and how it relates to sickle cell disease. I appreciate the refresher.)

This happens with SC trait but only 25% as bad… we get Rhabdomylitis from stress, dehydration, hot outside, extreme work out over time…it takes all of these things to happen for us. I am at my best when I drink 3 liter of water a day and more during stressful times, exercise and activities and in the heat.
I had a crisis in nursing school and was scary and severely painful stabbing but nothing like the actual SS sickle cell disease.
It happened again when I took a stressful nursing travel assignment in Colorado , I didn’t have adequate water in my hotel but I did drink 2 liters a day at least but it was so stressful, then I was invited to climb a mountain, it took 5 1/2 hours to climb to the top it was sunny but not hot and we had no water after we drank what we had….I then had a crisis and had to soak, rest and catch up on water.
Takes much more for us but it can happen, the pain won’t send me to the ER, over the counter will do and it passes in a few days depending on how fast you remove the stress and hydrate.
Be well and drink your gallon of water a day and more with heat and activities. If you are a SCT person.
I pray all the ppl with the disease can prevent permanent damage and live long stress free and mostly pain free lives ❤️

Omg, how happy I am to see you talking about this condition, I have sickle cell anemia and I got horribly sick bc of my illness for the very fist time when I was 8 months (I was told by my mom). I was first miss diagnosed with leukemia but finally bc how swollen my leg, feet arms and hands is when my mom was misinformed by oncologist that I would not live to see my 10th birthday, I’m 43 years old now. This is a terrible disease, I’m told I have the worst form of sickle cell disease. It has taken such a toll on me and my water, it’s so horrible with all the pain, iron overload and so many painful episodes

There is a loss of zinc in sickle cell. Millet is recommended for sickle cell because it is a good source of zinc. Supplementing zinc can cut the number of crises in half. Zinc picolinate is a well-absorbed form, zinc oxide is not. Look it up, ask your pharmacist, and ask your doctor.
