Just had my procedure today 1/23/2025. Had my epidural at my C6-7 right of midline. I was given anesthesia and let me tell you i didn’t feel anything at all. I remember laying face down, hooked me up to the machines and put oxygen in my nose. I don’t even know when they put the anesthesia in my IV. I guess i was out and had no idea. My point is. I had a great experience so far and I’m waiting to see when everything starts working. I hope it does. Thank you for this video. It helped me watching this as i did last night before my procedure.

My first serious neck injury was in 1998 when I herniated my cervical spine at the C5 - C7 area. I was in the most excruciating pain I had ever experienced for months as I visited doctor after doctor all of whom misdiagnosed my problem. Finally after eight months, I found a doctor who suggested I get an epidural guided by fluoroscopy. As this was my first injection, I was extremely nervous, but the doctor was very calming and extremely compassionate. What I remember most was when the doctor told me he was about to start the injection, and asked me to describe any sensations I was experiencing. The first thing I felt was a somewhat intense feeling of heat which quickly dissipated. By the time I was done describing this, I was told that the procedure was over and the doctor and nurses assisted me in rolling over and sitting up. Once upright, I was absolutely shocked by the fact that I was, for the first time in eight months, completely pain-free. It was miraculous. I was in so much pain when I laid down on the table and to suddenly be completely free of pain I was so physically and emotionally stirred that I noticed that a tear had started to roll down my cheek. The doctor saw this and lightly placed his hand on my shoulder and gave me a gentle squeeze to communicate that he understood what I had been going through and what the relief must have felt like. Over the years I had many more injections and managed to avoid surgery until 4 1/2 years ago when my injury suddenly spiraled leaving me unable to get around without the use of a walker. In the years since my surgery (multiple cervical laminectomies) I have managed to get back to about 75% of my normal baseline, but there has been permanent damage done. I experience pain and numbness in my feet and legs, as well as my arms and hands. I also still suffer from neck pain and must wear reinforced compression sleeves on my knees to aid my balance last I fall.
To anyone suffering from any kind of spinal injury who has thus far avoided surgery, I highly recommend getting at least one MRI per year to keep an eye on your condition. I feel strongly that if I had adhered to this, I might have been able to avoid surgery or at the very least, have my surgery performed sooner so that less permanent damage would have occurred leaving me in a better condition post-op.
Good luck to everyone out there! If you’ve had spine injuries, you know what others in similar position go through and it’s no joke.

Hi Dr Su you have done it again with such a great video explaining this procedure in a very simple way 👍👏
This Monday I had CT Guided C6 (left side) Nerve Root Injection at the Alfred Hospital here in Melbourne Australia. I believe that it was a transforaminal steroid injection as you explained because I was lying down on my back with my head turned to the right around 45 degrees I believe. They inserted the needle at the left side, base of my neck where they first injected some local anesthetic, then they performed the procedure. I was awake throughout the procedure. The problem is that after I left the hospital and the local anesthetic wore off, my C6 pain had increased dramatically, more than before the procedure. The pain has been excruciating when lying down, sitting down, when trying to sleep which was impossible. I was under the impression that I would be getting relief after this cortisone injection, but after 4 days I still feel very sharp pain starting from C6, going down the neck (it feels like a very intense sharp stabbing relentless pain), part of the thoracic spine, into left shoulder, arm, index finger, the groin and left foot when sitting or lying down. What could be causing this new added pain? Is this normal? Will the extra pain disappear and reduce eventually? I am a bit worried because the pain is stressing me out. Will the cortisone eventually work? Thanks.

I really appreciate your answer as I am in sever neck pain..here is my MRI RESULT
      MRI CERVICAL SPINE
      History: OA. Left radicular pain.
Results: The previous MB of 2021 has been reviewed. There is disc narrowing with loss of T2 signal at C4/5 and C5/6.
There is slight loss of height of cervical and thoracic vertebrae, unchanged since 2021. The spinal canal above C4 and below C6 is capacious. The upper cervical cord and medulla appear intact. The facet joints are normally aligned.
No compressive disc protrusion is seen from C2 to C4. The neural foramina are patent. At C4-5, the disc is mildly protruding. C5 nerve root impingement is not suspected.
At C5-6, the disc is protrading. There is no cord compression. Acquired foraminal stenosis is present bilaterally particularly on the right with decophyte formation, with evidence of C6 nerve root impingement. The C6-7 and C7-T1 discs are kormal. The C7 and C8 nerve roots are clear.
Comment: There is acquired foraminal stenosis at C5-6 bilaterally particularly on the right, but no additional high
grade stenosis is seen. There is no cord compression.

By the time my second appointment rolled around I was right back where I started, the second lasted a couple of weeks. The last two had no effect. About 15 seconds of pretty bad pain when he injected the corticosteriod (they will call it pressure lol), but neck feels better even the day after. I felt my arms getting real warm too
They said it was normal. I think that was the iodine contrast. I always flush bad.

    Hi Dr. Su,
I have a pronounced herniation (7 mm) I've been dealing with for 7 months. At the 6 month mark, I felt like my recovery had plateaued and would describe my pain as a 2 out of 10 with mostly just diminished range of motion.
I elected to take an ESI and the next day, felt really stiff with pain when my chin or neck was extended certain ways. That 2 out of 10 pain quickly turned to a 7 out of 10. It felt like my radiculopathy flared up again. Is this common? Would you recommend that I receive an additional ESI to maybe help get things sorted?
Thank you for your very informational videos.
