🥩 Pain docs must intimately get to know the “filet mignon”.
Aka the PSOAS muscle in the cow.
But you’re not a cow, you’re human.
Ya know how you have low back muscles on the BACK of your spine?
You also have low back muscles on the FRONT of your spine.
It’s called the PSOAS.
When it’s glued up by ADHESION, chronic low back and hip pain relief will stay chronic and annoying.
👉This is how we find, feel, and remove adhesion from the PSOAS.
Before we treat any muscle, we typically:
1️⃣ Confirm with a patient’s symptom presentation and testing (usually with kneeling hip extension) that there’s likely adhesion in this muscle.
2️⃣ Then we define our borders so we know where we are looking and use landmarks Ie. the umbilicus in this case
PSOAS lives about 2” lateral to the umbilicus
3️⃣ The adhesion specialist must sink in so he isn’t pulling any skin and generate tension IN THE MUSCLE, again, not pulling any skin.
Finally the assistant can move the leg so the specialist can feel the tension building and apply tension in the other direction to tear the adhesion.
After 2-3 passes, we recheck the test to see if we got it!

The psoas has to be the most misunderstood deep muscle in our entire body. It's not easy to stretch, and hard to massage around our abs and intestines
1000%. This problem is compounded worse by the fact that almost no one treats the nerve entrapments surrounding the psoas (femoral, genitofemoral, and nerve roots).
I hate this muscle it’s ruining my life since a whole year it’s so hard like I have a bowling ball underneath my abs

@@drchrisstepien I’ve been doing that with my doctor it does help also but it still isn’t 100% gone since whenever I’m stressed it does tighten up again :( I hate this muscle so much

what I have learned dealing with chronic psoas problem that never massage it too hard and deep. it only irritates it and makes it contract even more.

I just went through this last week and I couldn’t help giggling. 😂 Even though the swelling and adhesions are “very pronounced” according to my massage therapist. It was great I can’t wait for my next session.

Yes it could! If there is adhesion removing it would help but an evaluation would be needed, if you haven’t been checked or treated yet, seeing a doctor or therapist could be a good start

I’ve been suffering with debilitating back pain between L1-L3, lower rib cage mainly on the right side for over a year now. Bn seeing an osteo but never considered adhesion issues. Which makes a lot of sense due to having endometriosis & Crohn’s disease.
Have you gotten an MRI done ?
I use a a kids soccer ball, half deflated.
I just lay face down on it for a few mins and breath normally.
It does release the tension.
I have had chronic pain for 3 years. I had to leave my fix figure job. I’m on disability. I’ve been suicidal. They are treating my hip pain but I believe 100% this is the source of my pain. When my new physio found the spot, I had a rush of pain increase in my body and I cried out aloud, while feeling all the memories that created that pain all at once.
Can anything be done to permanently relieve that muscle?
I’ve been searching for a dr for a year who will inject Botox but no luck yet. My doctors have all said my papas pain is secondary but I know it’s not. Any advice?

How do you treat hernia in that region. Too old to have surgery.
Are you sure it's a hernia? Can you see it or feel the intestines? Many people have this problem who think they have hernia.
I am from western Canada and this is exactly what I have been deeply and painfully struggling with for a year now. Nobody can figure this out or what to do for me. A year ago I bought one of those Psoas tools you lay on bcuz it "was suppose to help stretch your psoas". Well I am pretty sure it did damage and nothing good.
Who would I see here to help? A sports physio therapist? I'm just so wanting some help. I used to be so active and my life has taken a complete 180.

I’m going to try an Osteopath which was recommended to my by a Naturopath…I’m seeking relief in that area (and more) from adhesions. My physiotherapist said it’s a good idea too. Fingers crossed 🤞 (I’m also from Canada)

Do you suggest Hip Abductor machines excercise in Gym to strengthen psoas muscle or does it compress femoral nerve?
Hip abductor machine has helped me greatly.

What kind of doctor do i need to visit for this? I have horrible adhesions after a hip arthroscopy went horribly wrong. I also have sever weakness in my right glute medius. It barely fires at al despite lots of PT. Can these adhesions cause digestive issues? I seem to have those too despite a nearly perfect diet.

@misterbulger call around to different massage therapy offices and ask specifically if they have someone who can do psoas releases. Massage Therapists do this all the time!

I have alot of pain in my lower back and in the top of the illiac crest. around 7 years ago i had a hard fall and impact to my right butt from snowboarding. my butt was numb for 3 days. the stiffness and QL/SI problems have stuck around and plagued me ever since. I also hyperextended my right knee around the same time and I was limping heavily for a long time. I am not sure if my iliopsoas is weak or if there is some sort of damage. (nothing showed up on x-rays and doc figured im fine) but sometimes if i try to pull my knee to my chest while standing, like youre going to put on a sock... i get a sharp pain in the side of my abs just above my illiac crest in the front of my hip. (i think it might be that psoas muscle) right in the area being released in the video.

2 weeks ago I injured that muscle by doing a move trying to catch something. I stopped working out. Im now feeling better and not much pain when doing stretches. But Im wondering can i continue weight lifting or I should wait more?
Don't do anything that increaes the pain until it can be pain-free.

My breathing has been shocking over the last 3 years. Lower abs suctions on inbreaths. Is it the psoas? My osteopath tries to release it but always clenches again and again. Is this technique better than lying down on the back?
If psoas isn't working, it's most likely a femoral nerve entrapment. We have a few videos about this

Isn’t there a risk to damage the intestines or any other internal organs when pressing there so deep?
Not at all.
They kinda slide out the way but I was taught to push in slowly so it doesn’t hurt so hard let the surrounding things relax as u go deepee
It may not be damaging but it’s probably uncomfortable. In Massage therapy school they taught us to go slow and in circular motions, so you kind of move the intestines out of the way. Think of spaghetti noodles and how doing that over noodles would probably move them out of the way more graciously than just pinching and pushing straight through

I had disc bulge but it recovered to a certain degree and now its back but it came after lower abdomen pain and now that i think of it, psoas might be the reason. Am i correct to think low back and low ab pain is connected through psoas?

@barefootrehab I have had low back pain for years that radiates over my iliac crest to my right lower abdomen. Sharp stabbing pains. I have had CT to look for hernia or appendicitis in that area because the pain gets so bad. The MRI shows some moderate facet narrowing in L3, L4 and L5 but the drs say my left side is actually worse and severe. I have no pain on my left side just my right. Could this muscle be what is causing my abdominal pain and if do I how do I get my Dr to address it. I have a hard time getting any of them to listen as they all say my left side should be hurting. Thanks
Yes!! It’s most likely this and or a femoral nerve entrapment. We hear horror stories all of the time from our patients. More and more adhesion specialists are popping up so we are coming!

Hi bro, i have a horrible pain when I bend my hip to the left ( so pain is on the right of my hip). Ever since I got injured in football, 2 days ago, it hurts when I walk forward ina specific way or if I bend my hips to the left. Is this because of my psoas or iliopsoas? ( I stand somewher and put my feet 4-5 inches appart, then I bend my body to the right and the lefts side of my hip is ok. But when I bend left, there is immediate sharp tugging pain in my right side hip area 😭. It also happens in my right hip when I move my hips forwards.)

Could be IP or it could be a disc or hip inflammation? Give it some rest and see how it responds
@@drchrisstepien hi! I'm pretty sure now, that I have greatly strained my hip flexor muscles on the right. There are certain movements that I make that I see others talk about on UA-cam, which cause pain. I can now self diagnose I guess and say that it's a psoas muscle injury along with my glutes. ( Not the big glute, but the smaller glute muscle section, ( whatever that's called ))
Is there any excercises or stretches you recommend me? ( All I have is a foam roller at home and my body weight ) ✌️
