  • Liam had been dealing with a lifetime of kneecap dislocations. He is an avid hockey player and needed to find his answer. Dr. Sabrina Strickland of the Hospital for Special Surgery in New York City sub specializes in treating kneecap instability, and now Liam is back to hockey better than ever.
    sabrinastrickl...
    Patella (Kneecap) Instability
In the front of the knee the kneecap slides up and down in a groove in the thigh bone. Sometimes it can slip out causing pain, swelling and cartilage injury.
    How does it feel?
It can feel unstable and in severe cases it can be difficult to fully extend the knee. Sometimes patients have to go to the ER to get the patella back into the groove. Often these injuries are accompanied by swelling and it can take weeks to months to feel normal again.
    How does it happen?
In some patients it happens without any trauma and at a young age. In others it can occur playing sports with or without a contact injury. It can recur especially when it first happens before the age of 25.
    How can it be fixed?
In most cases it can be fixed by reconstructing the MPFL (Medial PatelloFemoral Ligament) which is a minimally invasive surgery with a 4-6 month recovery. In patients who have suffered cartilage damage the surgery may be more complicated with the addition of a bone re-aligning surgery such as a tibial tubercle osteotomy and/or a cartilage replacement surgery.

Same kind’ve thing happened to me I was just lightly skating and then boom instant knee pain and I knew it wasn’t just a cramp. Trip to the hospital and 9 hours of waiting for my patella to be back in place and I was out of there. It’s been just over 3 weeks since my accident and I haven’t had pain since the first week, just feel unstable. I am losing a significant amount of muscle in my left knee, but my right one has gotten so much stronger already. My injury didn’t require surgery, but if I tear it again I will. I appreciate this video a ton because I didn’t have it as bad as Liam and wish him all the best in his hockey career!

Hi dr.Strickland, I am from Sri Lanka my farther is also a doctor.I am 17 years old.
My kneecap got dislocated last december 26th while playing Judo. I was immobilized for 6 weeks due to the fracture. Then i did PT and now i can walk and do lots of physical activities like washing cars, gardening, construction.I still cant run or walk downhill properly my kneecap feel really lose, like my kneecap is being wrapped by something to the knee joint. So i did a MRI, it says i have a partially torn mpfl😖
I dont want to play judo again.i want to become an engineer at air force but i think i wont be able to participate in the cadet training because of my knee.i live in a third world country i have to be able to protect me and my loved ones i need my knee.
So i still didnt went to the surgeon because he went to a holiday. Next monday i will visit him. My farther told me i will have to do a MPFL repair surgery.
Will my knee will be alright after that?
    - My range of motion is better than my good knee.its better than the most of people. like my feet can literally touch my buttocks.
    - little bit of pain
    - didnt dislocated for 8 months

Dear doctor Strickland. In sept 2023 an idiot hit me in traffic and almost killed me. Result: acl, lcl and a fragment in my fibula came off. First surgery in oct 2023: they put the fragment back + lcl reconstruction. Then i had to train for4 months and a half and then acl in march 2024 because during my pt i had a lot of instability, pain when i walked and also experiencied this weird rubber band feeling when i walked. So acl reconstructed in march. Now i am 4 months into pt and my muscles become stronger and stuff. However... i still have this elastic feeling that starts from my lcl and going to my tibia it seems. And its really annoying and instable when i walk. Have you ever came across such a casus? Here in Belgium we have a very good health care system and doctors but still. It is interesting to ask other surgeons their opinion.

That sounds like a difficult problem. Unfortunately, it's impossible for me to sort the cause of those symptoms without an MRI and evaluating your knee in person. It may be just scar tissue that is causing this feeling. Hope you continue to improve and glad you survived the accident!

My son is a high level competitive high school athlete, so I like the idea of using donor tissue vs grafting from his own healthy tendons. However, especially because of his age, I worry about disease transmission from donors. While highly unlikely, I believe I'd read the odds are in the ballpark of 1 in 160,000 of such an occurrence. After 8 dislocations of my right patella from hockey I finally had MPFL surgery 2.5 weeks ago. At age 53 I’m sure it would have been wise to have done this 15-20 years ago after the 4th time. Recovery is slow but I’m through most of the pain now and working on the ROM and building the muscles back up. Bending at 45-60° now with a bit of pain at 60° and quite tight.
