Rigging contractor 40+ years--here's my take on it. What occurred: A1. Far side on left end failed first, causing the bar to upend. A2. Near side on left reacted, sliding & cutting sling at outrigger beam. Mistakes made: B1. Basketing the sloping part of outrigger beams makes slings slide easily. B2. Did not monitor level lifting with cranes (B1 issue). B3. Shackles with basket slings are too small to properly contain them. B4. Sling size, condition, and proper protective pads???.
The sling failed I wonder if they checked to see if the sling had any knots or tears or if the grabbed the wrong sling and it wasn't rated for the pick
Look at the video image by image. At 2:51 you can see a rope forming a double loop at the left-hand corner of the hoist. The rope breaks at the bottom, and the two broken ends rapidly shoot up. Astonishingly. the load does not drop at once; probably the rope at the other side still holds. When that side breaks by overload, the left end of the load starts to drop. The camera has recorded that perfectly; no further evidence on the cause is needed. The crane drivers made no errors. The rope was either of a type too weak for the task, or a correctly chosen rope with an unnoticed defect.
Even after watching this three times: 1) I don’t understand what is being lifted 2) It APPEARS as though there are still lines attached to the underside of the lifting bed (if that’s what it’s called)
Exactly and you know what they need more rigged support is like to much weight and no enough rigging mmm stupid people... I am a ironworker and always I used more rigging then needed cuz that’s safety always be prepared
it appears the load could have been disassembled to offer lighter, more easily manageable, less awkward pieces where u fly only the base frame/wheels portion into place first, then the cab in a seperate load second before bolting/wiring it all back up....i'm not familiar with or fond of the dual-crane pick so i'd have rigged it up on a single hook(cab sits in the way of where a hook/shackle would be positioned if arranging a single hook setup with 45 degree angles on the corner chokers for the maximum stability/support/weight distribution).... they did a setup where each corner is rigged right beneath the attachments and this arrangement offers no support for the area of the load between the corners and they should have used steel chokers rather than nylon slings and inserted padding/softeners (such as dense rubber matting to eliminate metal-on-metal stress/friction on the load) between chokers and sharp frame edges and, depending on weight and space provided, maybe a couple big ol' turnbuckles to basket the load and reinforce/connect the chokers.... lastly, same side would tie boelin knot tag lines held on to by none other than competent union tradesmen...perimeters surrounded with red danger tape isolating the entire work area to keep dumbfucks like this cameraguy away from these critical lift scenarios

Im surprised that people were permitted so close to the lift; no one needs to be nearby you couldn't pay me enough to be near this lift , in the danger zone.it falls, all it takes is a little piece of steel or concrete to be hurled out, go righr through a human body like a bullet
My father experienced a huge accident in that way. 100% overload on a 100 meter hoist 😱💥 Unbelievable no one hurt, court case took more than two years 🤷♂️
you are right. a brand new harbour mobile crane(the same like those who lifted the new(smaller one)those crane are really expensive! i work at this company(liebherr in 2008)
We are taught to ensure the hoist braking works properly. Also, environmental variables, such as wind can pick up during a lift. Must ensure the load is stabilized before proceeding.

REMINDERS: before lifting anything you must have a computation for the sizes of cables, slings, shackles, Radius, Boom length, and Boom angles etc.. maybe the operators of this crane are not Certified as a crane operator,
They had to raise the load high enough to clear the ships deck.It takes time to lift especialy a load of this weight. Ive been on large ships and watched them load private yachts onto them and ive never seen them do 2 moving actions at once. The cranes might not have enough hydraulic power to lift and swing the load at once. Looks to me a strap gaveway.

I know what happened. They didn't have enough balloons on the left side to take enough weight off the rigging.

nylon on steel.. just dumb.. no amount of softeners would have helped with that weight.. bet it has lift points for steel shackles.

Look at the video image by image. At 2:51 you can see a rope forming a double loop at the left-hand corner of the hoist. The rope breaks at the bottom, and the two broken ends rapidly shoot up. Astonishingly. the load does not drop at once; probably the rope at the other side still holds. When that side breaks by overload, the left end of the load starts to drop. The camera has recorded that perfectly; no further evidence on the cause is needed. The crane drivers made no errors. The rope was either of a type too weak for the task, or a correctly chosen rope with an unnoticed defect.

You can see the sling/strap on the left attached to the spreader has a twist in it. Only one but that might have been the difference.
