If you hadn't noticed, everytime it was his turn, the arrows light up (mainly because of the fact that previously, he was the original boyfriend). In the song "Genocide", Tabi is like boyfriend, where he has to get perfect notes to regain health. To sum it up, it's a "Player vs Player" type battle.
Agoti's case is more for visual appeal, it doesn't have any implications or any reason to be more than "it looks good". Tabi is different because it correlates with the fact that he was like you, Gf's boyfriend, & it's bugged also. When it's Tabi's turn in hard mode, it's supposed to drain close to 3-4 times more than in the video (~70/80% health drain, so BIG boy drain). If you watched other let's play where it isn't bugged, then you would notice it drains a lot faster (too much to be "smoke in your lungs").

I’d like to figure out how to manipulate the health bar like that for my own gimmick, like a thing where during a certain period, you can’t recover health even if you hit the notes, but it’s better to hit them than to ignore them.

Well, in this video alone, Tales from the Raveyard doesn't. While you wouldn't expect it at first glance, it's surprisingly wholesome - the witch isn't evil, she's not doing any harmful magic or anything. She taught Boyfriend the basics of magic, summoned peaceful ghosts with his help, and then the two did a final duet together to entertain them. She even considers Boyfriend and Girlfriend her friends by the end of it. The same goes for Garcello, I guess, technically - he doesn't have a seizure, panic attack, or mental breakdown; he just fucking *dies.*
Hex wasn't evil either, he just wanted a friendly battle and you even save him from a virus (sure that virus may count as one of the things mentioned) but at least Hex was nice all the way through

I don’t know much about the Tabi mod but it seems very high effort. It’s a shame that it seems to just be another one of those note spam mods that have a last song that’s literally impossible for any normal human being to play.

Whitty, even though he’s literally a bomb, is someone who neglects conflict and would rather be left alone to protect himself and the people around him than to start a fight over something minor. Tabi on the other hand, is someone who will do anything, even if it involves killing innocent people to get his revenge on GF. But there is a possibility since they both got some severe anger for GF and BF. Whitty was pushed and harassed by BF to the point on going ballistic and Tabi was fucked (metaphorically) by GF to the point of being a skull figure with no body.
