Weir looking different is further explained in the show. She looks different because she just used the template that was already in the replicator machine: Fran.
The plot hole is why she looks like Tori Higgins in "Be All My Sins Remember'd" and then never looked like that again when she was among the other replicators in the flashbacks.

In Weir's first appearance she was a blonde lady played by a different actress, so this wouldn't be the first time she's looked different.
Keep an eye out for Major Motoko Kusanagi... the joke set itself up.
The one on the bed isn't Weir. We're seeing things from Weir's POV. Then they use the Fran body for her later.
Except why would he speak to the one on the bed as if she were Dr. Weir?
