Spoiler space, to protect those not familiar with the original story:
I've seen a couple of people say that Keiichi is not paranoid yet, but that can't be true...otherwise, he wouldn't be seeing Rena's eyes go demonic. So he is definitely in the grips of the disease, as even Rika stated in the original VN that his two-day absence was tragically enough to trigger its symptoms to start manifesting in the Onikakushi fragment...normally something that wouldn't happen, but Keiichi basically rolled a crit miss on his "do not go berserk" check. Even after he sees Rena "transform" on their way home from school, he still tries to rationalize it in his head, though all of that crumbles the exact same night. At least when following the original storyline, Keiichi's paranoia isn't supposed to *really* ramp up until the phone call that Rena overhears, which we just ended with in episode 3. Then the next day, there's the conversation with Ooshi that directly implicates everyone in the previous years' incidents...and that's when he finds out about Rena's history (though they jumped ahead with that part, probably to help with the pacing).
This adaptation definitely has some things going out of order or just plain missing, as you mentioned, but I think we're going to see some of the key elements appear in episode 4. For example, we really can't have this part of the story without the ohagi scene...which kind of relies on Keiichi going back to Angel Mort with Ooishi since him freaking out in public and being questioned by Mion was an important part of it. After the ohagi scene, there truly is no turning back.

Is it possible rena's eyes are either A) artist interpretation of her looking odd in reality or B) literally odd due to supernatural possession or interference?

@@DallenMalna The eyes would not be artist interpretation alone, since the scene was done the same way in every presentation: visual novel, manga, 2006 anime, and 2020 Gou version. They are intentionally done that way to hint at Rena's inner darkness, but the problem is not with Rena herself...at least, not entirely. In both Onikakushi and future arcs, she exudes a similarly creepy atmosphere when talking about Oyashiro-sama's curse, but outside of that first arc, she never appears what I could call "demonic" while doing so. As Mion explains in Tatarigoroshi after Rena rambles madly about Satoshi and the curse: "Rena doesn't take anything regarding Oyashiro-sama's curse lightly". But until the very end of Onikakushi, Keiichi never mentions the curse to Rena, yet we have the creepiest eyes she ever displays simply from his accusation of her hiding things from him. The real issue is perspective, and as we know/will see, Keiichi's is very rarely the best...

@@Shuukuriimudaisuki-sama I'm still not entirely convinced keiichi is infected but you make really strong points. I'm excited to see how it pans out soon!
