When the patients upper body becomes wider than the lower body. You get what he means, the body fat upstairs is wider than the 'ass' part. So, picture comedian Chris Farley. this still surprises me because my mind still thinks it must be heavy smokers, drinkers, people who don't eat their vegetables but no. No it's still actually 'obesity', its still 'fat to muscle percentage'.
It's too much strain on the heart, and the inevitable metabolic syndrome and diabetes is too hard on the organs. My family knew a very large girl who died in her early 30s from a case of soda a day. No other vices but sugars and bad food.

It's heart and internal organs, things i'd never thought about. however, the carrying of weight is actually one of the direct factors and we knew a gal, only 23 years old, obese, her feet were so busted with bone spurs and ankles so crushed they were as if the age of a '74 year old woman'. that was directly the weight as she walked. What happens? Then the obese avoid walking or they simply cannot as ligaments and joints wear out. Then exercise is over and so is expending calories and that's the deadly slope. There are other things that surprised me but one is, literally, physically, the weight of their chest when if they're sleeping. to my mind this can't be much but someone who's already heading for a heart attack, actually the weight of the fat impeding their breathing is a real, no-joke, serious contributor to ending them. btw I'm one of these genetic lotto types where I can definitely get fat. I can also put on a lot of muscle if that's what I was training at. A relative, who is dangerously obese will routinely gripe during holiday get-togethers "yaaa. you're lucky though, you don't have the fat genes I got stuck with". Wrong! not true. I could EASILY get obese. It's just that I realized its actualy one of THE single most dangerous factors to health so I just remind myself to take a break on the pepsi and extra cookies. *and yep, pepsi, those sweet drinks, wow can I get fat on those fast!

I’m a union electrician and will retire with with more than 3 million just from my stock investments over the last 20 years and it wasn’t like I was throwing hundreds of thousands of dollars a year into my fund. More like 10k a year.
