Until finally, you hear so acutely every little sound feels like a headache is coming on and the shock of feeling fresh air on such a raw, tender spot is sharply felt. A spot hat you can't reach!

That's right. Removing keratosis obturans is extremely painful. We loosen the earwax using the hook while immersing the whole ear in anasthetic fluid to manage the pain. Thank you for your kind word despite all the hate comment 😁

A scoop is used for wax. These are ribbons of dead skin, a condition called keratosis obturans. You can tell by how it appears to be "encased" in a white, puffy layer which is dead skin that has not oxidized yet. The darker center is full of very tightly packed ribbons of dead keratin. He's slicing thru the ribbons with the hook tool so he can suction them out.
Absolutely, I completely agree. Using these kinds of tools is essential for gradually removing earwax bit by bit. It's important to do this slowly to protect your eardrums.

The camera always makes the ear canal look so big that you start thinking 'why don't you just grab a spoon and scoop it out!'. Amazing skill to get all that out without completely destroying the ear canal.
That was some ear wax. Would it not have been more effective to have used the jobson horne on this one, less stressful on the patient too? Tho bet the guy was over the moon to get his hearing back.
That must have been such a relief for both of you when it was loosened enough to be able to remove it; your arms must have been aching from the amount of digging & excavating you had to do and the patients ear canal looked so sore, itchy and painful, but it must have been such a relief for it to be gone. I bet everything was incredibly loud once their ear was unclogged!
Oh my God, how painful was that extraction! I hope you could use local anesthetic here. That Keritas Obrans at 9 O'clock was on it's way to becoming a canal Kleciotoma (spelling ?) or worse. I hope this person comes in every three months or so till the ear can clear itself naturally or it's a quarterly cleaning. I am sure their hearing return was great after the pain and itching subsided. Great job on a tough clean out. I think you speeded up the video at a few points, and it took longer than 15 minutes to clean all that keratin, then the surrounding skin in there extracted. Very good work.

I also wondered why the person continued to use that particular tool and not use suction to get the loose pieces out. I believe that it would have made it easier to see where shedding skin was still stuck to the canal. However I realize that every person does ear wax removal differently.

I'm not a doctor yet from what my observation is that the wax was too soft too pull out the normal way. Even with the correct tools. So he used the skin as a blanket to pull it out. The wax was soft like peanut butter

Why not use the standard size Zollner(sp) to remove much of the loose debris?

Jobson is too huge to use. Bleeding is common in keratosis obturans removal
