A scoop is used for wax. These are ribbons of dead skin, a condition called keratosis obturans. You can tell by how it appears to be "encased" in a white, puffy layer which is dead skin that has not oxidized yet. The darker center is full of very tightly packed ribbons of dead keratin. He's slicing thru the ribbons with the hook tool so he can suction them out.

Oh my God, how painful was that extraction! I hope you could use local anesthetic here.
That Keritas Obrans at 9 O'clock was on it's way to becoming a canal Kleciotoma (spelling ?) or worse.
I hope this person comes in every three months or so till the ear can clear itself naturally or it's a quarterly cleaning.
I am sure their hearing return was great after the pain and itching subsided.
Great job on a tough clean out. I think you speeded up the video at a few points, and it took longer than 15 minutes to clean all that keratin, then the surrounding skin in there extracted.

I normally don’t comment but this is clearly repeating video to make this last longer and it seems unusually cruel to the patient to have to endure this one second longer than necessary

You need to know that an ENT doctor has spent a decade in training with hands-on experience. Maybe you should dedicate the next 10 years of your life becoming one. You think watching a few UA-cam videos here and there makes you an expert?
