So just to be clear, in respect of the Google Respondent Ad setup (5:10), do they generally 'always' use 'one' of the top two headlines, in conjunction with a mix of the headlines below the first two? I ask, because I've never really understood how it works, and it's always worried me that they may mix up headlines together that are repetitive (where you have different variances of the same phrase in a few of the head lines e.g. 'Free Quote', 'Free Project Proposal', and 'Free Project Proposal & Quote'). Additionally, the same question goes for the descriptions? :)
Google tests everything and sometimes it does repeat itself but it very quickly corrects itself. I wouldn't worry about unless you had a very low click through rate.
When creating 3 responsive ads per ad groups, should all ads have different headlines/descriptions from each other with a different sub-goal? For ex: I'm a kid's toy brand, should I create an ad each focusing on a product range (1 for boardgames, 1 for video games, and 1 for puzzle games)?
I recommend changing as many headlines as possible that way you give google the best chance of finding a winning headline. For the example of your toy brand I would recommend building out separate ad groups for each one of those products and then creating multiple ads for each product

Thanks for the Video! Do you pin any headlines? For example, do you pin the brand name in headline 1?
No problem and I normally don't unless there's a reason to, like the brand name is a reason to buy (think super high end brands).

Please explain how many keywords you have for per ad group.
