I've watched a few of Neil's videos and thought yeah this guy seemed to know his stuff but watching him actually fun through a fit, on a complex issue is, frankly, amazing. Even more so because i've suffered a very similar issue. Having had several gout attacks the structure of my feet changed, causing them to differ in length, resulting in instability with my walk / gait. This in turn caused the tracking of my foot, knee and hips to become misaligned, resulting in knee pain in both knees.
I went to see a osteopath for help before I knew any of this and they were absolutely useless, a bike fit was much more helpful.
I'm still working through trying to get everything back to normal but thing's like arch supports and hip exercises seem to help
HI @KarlGuillois, Thank you for sharing your case study. I have the same problem as you it seems and on the same side too. In my case, I was riding in a lot of pain as my body can no longer compensate due to aging. Good thing you found Neil. I have applied everything i learned from this video and for the first time in many years, i can ride without pain and sit squarely. Thanks

As an avid cyclist and current sports student (approaching 3rd year Sport Therapy & Rehab BSc degree) I'm always interested in what Neill does within his bike fits and and his postural analysis. I find my knowledge forever expanding. Loved this video and the way everything was described & explained. Takes great dedication to fly all that way Karl, chapeau to you. Really interesting to see a short leg/short arm/tall torso fit. I'm 193cm and have crept down from 61 to 58 and now 56cm frames searching for comfort.
First time I have EVER heard someone recommend lowering the front end to help shoulder/neck pain. I had to do the same and I’ve been preaching it ever since. Very interesting.

Why can't all the bike fitters like Neil, so enthusiast to make rider better on the bike. I had 2 bike fits, 1 Guru and 1 Specialized Retul...I have yet to use both measurements a total waste of time and money.

I'm the opposite. Right foot is arched, thus smaller 5mm. Left hamstring tight. Sitting all funky (mtb xcm/xco). Sometimes I manage to sit square and the legs spin so much easier... position doesn't last though. Wish Neil was closer.

Who do we know that charges a lot of money and only sells lake shoes in London 🤔 can't believe they didn't stagger the cleats, seems like bike fitting 101 to me.

James from Bicycle, the guy featured in the (great) bike fitting series on Cade Media UA-cam channel. There are other bike fitters working in the shop, but they all use the same methodology. Sometimes you just miss something which seems evident afterward when you know what was the « solution ».
Rrally interesting to watch this. Neil is amazing. I myself have some asymmetry issues that have not been fixed/helped by two respected fitters in the UK. Im now wondering if its worth going to a 3rd fitter or just to hang up the wheels. Its frustrating. Love these videos Neil.

This is pure gold to me. I have a foot that is 8mm shorter than the other, I wonder if that is contributing to me sitting wonky ?
Well, it's worth a try to stagger the cleats by 4mm or so and give it a go, if I get wonky to the other side I'm on to something.
Amazing literally the same problems I'm facing with my left leg! Thought about the smaller shoe but it is not cost effective right now. I have learned so much from these videos, I'll pay Neil a visit as soon as I win the lottery! would love to see a follow up session with Karl to see his results in the future.

I’m just wondering, BEFORE going to an expert Bike Fitter like Neill, would several visits to a Chiropractor and/or Physiotherapist to realign and correct the cyclist’s spine/hips/joints etc make the bike fitter’s job easier and quicker? Keep the videos coming!

Crazy to go to a 47, but make sense. I used the Hinault method, from his book, and went to a 53 with 53 toptube (from a 58cm I think), much more exposed seat tube, but the reach was great for flat, but no good for long hills and mountains.

I got the same problem with different feet sizes. My right foot always gets numb, no matter where I set the cleats - I mights just end up needing to buy two pairs of shoes just to use one of each and set the cleats in the right position relative to each other.
another awesome video and not surprised that someone would travel so far ...i suppose part of the problem is knowing how to find bike fitters operating at this level (we know not everyone is a youtuber or on socials) .

Interesting I have the same issues with my feet but with a 15mm different, add in muscle weakness and instability with my right hip due to undiagnosed hip dysplasia and I'm finding it difficult to stop my knee pain. I never buy two different size shoes the smaller size never seems to fit my smaller foot 🤷. Podiatry gave me high arch insoles for both feet but my flatter left foot isn't tolerating it for long. I'll keep fiddling with bike fit 🤞I get there eventually.
It depends - I dropped my bars by over 20mm and found lower a lot more comfortable including riding in the drops but a 10mm longer stem would be torture on long rides.
In this situation, the 10mm longer stem is to compensate for the shorter reach as he goes down to a 47cm frame.

Lol, honestly though, really amazing video. Both amazing seeing how simple changes make such massive foundational shifts, but also very interesting to see as someone that also seems to have some left-side imbalances. In my case, I'm just getting started cycling, and my issue is pronation, not supination. So I realize I shouldn't be replicating anything here specifically. But it was super interesting to realize that my left foot being longer than my right might be caused by the slack arch on my left side - in my case, maaaaybe 5mm at worst. In Karl's case, 1cm is just stunning.
I think personal notes of importance to myself would be to continue working on building stability by making sure I'm engaging all the right muscles - as example, using resistance bands around the knees while squatting. And maybe see what I can do to train hamstring engagement to balance out the quads. Also, just in general, the importance of strong arches (and adequate support on the bike) to keep things balanced.

I’d like to see Neil do a pro mountain bike fitting.
I've been navigating a slight disagreement between my bike fitter and my physiotherapist. And there's enough of a difference in my feet length -- nothing this radical of course, but the same basic cause, a collapsed arch lengthening one foot a fair bit and resulting in about a 3mm-4mm difference -- that this might just resolve it. So the cleat on the left foot is going to go just a touch bit forward as they're both all the way back.

After watching many bike fitting videos, I'm curious about the potential limitations of bike fitting. While some similarities to pseudosciences such as e.g. chiropractic or homeopathy exist (focusing on individual needs rather than standardized approaches), I recognize the need for more evidence before forming a strong opinion.
Here are some specific concerns I have:
Focus on accommodation vs. technique: Do some fitters prioritize mechanical adjustments over teaching proper riding technique to address body quirks?
Subjectivity in evaluating fitters: Is there a clear way to distinguish between qualified and unqualified fitters?
Universality of anatomy claims: Is the statement "everyone's anatomy is different" an oversimplification that might downplay the effectiveness of standardized fitting principles?
I'd appreciate any resources on evidence-based critiques of bike fitting. As the public becomes more critical and science-minded, it will be interesting to see how bike fitting practices evolve to ensure their effectiveness.

I'm surprised he didn't clock you when you said Camembert cheese then Tasmania haha. Even though his obe foot is 10mm shorter the cleat position should be the same position relative to the shoe as the shoe size had changed? Unless the foot anatomy is different eg ratio of bone dimensions to foot of course. Ive heard of riders with different metatarsal heights that they correct with arch supports molded to equalise the feet
I have this exact issue, also made worse by being hit by a car. How the hell do you get the hamstring to work again? I reached the shim under left foot stage which goes some way to masking the problem but still lop sided in both position and power output.

your comments in the last minutes are so true - and a bit demoralizing as well. I have seen so many bike fitters giving advices which seam completely incomprehensible to me! One of the big German cycling magazines made a story some time ago, sending the same guy with the same bike to different bike fitters and they "corrected" his position totally different, really in exact opposite ways (partly some cm off!), not two of them came to the same results. Not very encouraging to go to one of these guys... I'm sure Neill would find improvements for me as well, but I assume most guys here would rather make things worse...☹

I had been wondering for some months how long it would take before Neill eventually shifts from narrow pointy shoes to barefoot ones.
I see he had a wedge in his shoe I put wedge in mine but I need it on the opposite side when the when I'm I walking or running is that sometimes the case? Our feet mechanics are different when cycling correct?

We are in the peak of the computer fitting times.
But the computers cannot adjust to each person abnormailies, and thats the reason why lot of rider not satisfied after fitting from a fitter that trust the computer and has no real fitting exoerties and expeerience.
you still need a human skills to make it good!

Is there value in just embracing the imperfections in our bodies, providing its doing no serious damage. Our amazing bodies adapt so well
That is good for short rides but not crazy stuff like tour de france. Riders that don't have to compensate as much will be just better.
I wish my local bike fitter put that much effort into it. Not even close :(
