    Hi Jay. I'm a nurse and planning to take OET this year. I have seen some sample case notes where past medical history shows a list of medications for past medical problems related to the present medical problem. Is it important to list all those medications (there are a total of 5 meds)?

    If we put “at the age of 2 weeks “ in the end of sentence then it would be right?? Like I answered he had stroke which resulted in cerebral palsy at the age of 2 weeks!! Anyone??

    With regards to these sentences "Jane lives alone but has a rehabilitation aide who handles her shopping and errands." " Although Jane lives by herself she does have an aide who provides home support." I am a bit confused with the use of WHO on those sentences, are we pertaining to the aide (which is a thing) so , is it more appropriate to use WHICH to refer to the aide that helps Jane? Thank you. :)

    I want to know when you are writing Dr. Martin we need put a dit after Dr,
    Second thing is when you are writing salutation we need to put comma after it. For example Dear Dr. Mart, . Because book says you don’t need put a comma here and if you put a comma here you should put a comma after yours sincerely. What do you recommend??

      Currently , Jane is unable to move around independently , hence she is using power wheelchair to assit her in the mobility.
